Objective:To evaluate and analysis clinical efficacy and its influencing factors of anterior cervical vertebral subtotal resection in the treatment of segmental ossification of the posterior longitudinal ligament of the cervical spine through clinical observation and case follow-up.And further to explore the recognition process, pathogenesis clinical characteristics, surgical treatment method and the advantage of anterior cervical surgery advantages,and provide reference for clinical treatment of ossification of the posterior longitudinal ligament of the cervical spine.Methods:A retrospective analysis was made on 37 patients who accept anterior cervical vertebral subtotal surgery from October 2010 to March 2015 in Affiliated Hospital of Shandong University of Traditional Chinese Medicine.Using the the Japanese Orthopedic Association improved JOA score rate to assess surgical efficacy.According to a number of factors may affect surgical efficacy,such as course of the disease,spinal canal occupation rate,for statistical analysis,reveal its impact on surgical efficacy.Result:Patients with preoperative JOA score from(10.30±1.54)to(14.81±1.15),According to Hirabayashi method to assess JOA score improvement rate at the end of the period time,the superior rate was 91%,the last follow-up JOA improvement rate(66.33±13.24)%,Surgical curative effect is good.The statistical analysis showed that patients with clinical course(p<0.05),cervical spinal cord MRI signal(p<0.01),spinal canal occupation rate(p<0.05),ossification morphology(p<0.05)were all associated with surgical efficacy,trauma in the early history of follow-up(p<0.05) associated with the surgical curative effect, the last follow-up is not statistically significant(p>0.05).Conclusion:Anterior cervical vertebral subtotal resection was a distinct and effective way for the treatment of segmental ossification of the posterior longitudinal ligament of the cervical spine. patients with clinical course,spinal cord MRI signal change,spinal canal occupation rate,ossification morphology all have of varying degrees of impact on the efficacy of surgery.
