| Objective: In this study,Pingle setting manipulation is used to reset the young adult femoral neck fracture;Individual 3D printing navigate is designed through computer,hollow nail is driven into the femoral head through navigation in an inverted isosceles triangle,and observe the clinical efficacy of pingle bone setting manipulation combine with 3D printing navigation and Cannulated screw fixation treat young adult femoral neck fracture.Methods:30 cases met the inclusion criteria were randomly divided into two groups according to the ratio of 1:1.Therapy group:Personalized Navigation is designed according to the patient CT data and printed out before surgery,then Pingle setting manipulation is used to reset the fracture,ultimate,hollow nail is driven into the femoral head through navigation in an inverted isosceles triangle.Control group: Pingle setting manipulation is used to reset the fracture,and hollow nail is driven into the femoral head through the surgeon’s experience in an inverted isosceles triangle.The date such as blood loss,fluoroscopy times,operation time,needle times were recorded.And the X-rays were taken after a week,then Garden index and cannulated screw arrangement layout is analyzed according to this X-ray film.Then the X-rays were taken after three months and six months,and the healing of fracture were analyzed according to this X-ray film.Results:Therapy group operation time 64.53±12.47(min),control group operation time49.80±16.23(min),there are statistically significant(P<0.05).Therapy group blood loss 91.34±9.9(ml),control group blood loss 27.34±13.35(ml),there are statistically significant(P<0.05).Therapy group fluoroscopy times 21.4±3.33,control group fluoroscopy times 48.74±4.98,there are statistically significant(P<0.05).Therapy group needle times 11.67±3.43,control group blood loss 28.47±3.78,there are statistically significant(P<0.05).Therapy group Garden index gradeⅠ13 cases and gradeⅡ2 cases,control group gradeⅠ12 cases and gradeⅡ3 cases,there is no statistically significant(P>0.05).Therapy group cannulated screw arrangement layout 13 cases is inverted isosceles triangle and 2 case is non-inverted isosceles triangle,control group cannulated screw arrangement layout 9 cases is inverted isosceles triangle and 6 cases is non-inverted isosceles triangle,there is no statistically significant(P>0.05).Therapy group:fracture healing in three months 13 cases and fracture nonunion in three months 2 cases,control group: fracture healing in three months10 cases and fracture nonunion in three months 5 cases,there is no statistically significant(P>0.05).Therapy group:fracture healing in six months 14 cases and fracture nonunion in six months 1 cases,control group: fracture healing in six months 13 cases and fracture nonunion in six months 2 cases,there is no statistically significant(P>0.05).Conclusion: Through the use of Pingle setting manipulation Femoral neck fracture Garden type III are well reset.It is fast and accurate to driven the hollow nail into the femoral head through navigation in an inverted isosceles triangle.And also the use of navigation could reduce the iatrogenic trauma,the requirements of surgeon`s level,and the occupational hazard to the surgeon and operating room nurse. |
