Objective: To investigate the efficacy and safety analysis of knee intra-articular injection of tranexamic acid on bleeding control of total knee arthroplasty.Methods: From October 2014 to December 2015, 90 patients were treated in Shandong hospital of CTM.Patients were randomly divided into 3 groups, group A control group of 30 patients(without using tranexamic acid); Group B the intravenous drip group of 30 cases; Local injections of group C group of 30 cases, are used in autologous blood transfusion device. Record relevant data to calculate hidden blood loss and postoperative total blood loss, knee range of motion(reference Appendix), were analyzed.Results: After the loss of red blood cells in an amount(including the loss of red blood cell volume recessive, dominant red blood cell loss, total loss of the amount of red blood cells), blood loss(including hidden blood loss, the dominant blood loss, total blood loss), B, C group significantly lower than that in group A(P <0.05), no statistically significant difference among B, C group. 3d after to maintain the level of hemoglobin, red blood cell count, hematocrit B, group C was significantly higher than group A(P <0.05), no statistically significant difference among B, C group. Blood transfusion after surgery, postoperative 3d again transfusion, blood transfusion with allogeneic blood transfusion again B, group C was significantly lower than that in group A(P <0.05), in which the group C minimum. The results show that local intra-articular injection TXA method can effectively reduce TKA perioperative bleeding, especially postoperative drainage and transfusion rate, postoperative complications incidence rates nearly three postoperative ultrasound in all patients found no deep venous thrombosis.Conclusion: Tranexamic acid injection can significantly reduce blood loss in total knee replacement surgery patients, postoperative blood transfusion, does not increase the risk of thrombosis and other complications.
