| The incidence rate of the lumbar facet joint degeneration and low back pain in the crowd is very high, it influences the quality of life,and increases social burden. The clinical statistics shows the rate of lumbar facet joint degeneration in helicopter pilots who have low back pain is hige. Some studies show that stress is an important facetor of lumbar facet joint degeneration. And the severity of degeneration is positively correlated with contact time. This study with an eye to the correlation of vertical vibration with rat lumbar facet joint degeneration and low back pain. The research conduction is of potential value to intervene in the occurence of low back pain of helicopter pilots.The study set the rat lumbar joints as the research object. Through loading 5Hz to 60 Hz vertical vibration on rats, to simulate the vertical vibration environment of the load of human lumbar facet joint. After the vibration period,according to the result of cartilage pathological staining and immunohistochemistry and the changes of content of cartilage composition and cytokines,the level of degeneration of cartilage is observed. From different levels and angles to explore the influence of long-term vertical vibration on lumber facet joint degeneration and the effects on low back pain.The experimental result shows the articular surface in control group is smooth, translucent and white, it has no crack. The joint edge is neat, it has no osteophyte. The colour and lustre of joint surface in experimental group is dim, transmittance drops, the edge of joint is rough. The degeneration occurring in superior articular surface is more serious than that in inferior articular surface. The degeneration of cartilage in 60 Hz vibration group is more severe than that in other groups. The result of HE staining of control group shows cartilage level is clear, tidemark is continuous, cartilage cells arranges orderly, middle tier "back-to-back" cells is visible, no cell clusters, cytoplasm and substrate staining is uniform. With the increase of the frequency, the cartilage layer disorder and get uneasy to be distinguished, the tidemark is fuzzy, the permutation of cells is disorder,occurring cell clusters, uneven colouring matrix, and lighter dye.The Mankin score of the control group is(0.80 + 0.63) points, 5Hz vibration group:(3.10+0.57) points and 20 Hz vibration group:(5.80+1.23)points,60 Hz vibration group:(8.70+1.25)points. The immunohistochemical results show that the average optical density of COX-2 and PGE2 in vibration group was higher than that in control group. And with the vibration frequency increases, the difference is statistically significant(P<0.05). The average optical density of CII between control group and 5Hz has no statistical difference(P > 0.05). The average optical density of CII in 60 Hz group and 20 Hz group decreases obviously than that in 5Hz group and control group. The number of apoptotic cells in articular cartilage of the vibration group is significantly higher than that in control group, the difference is satistically significant(P<0.05).The Experiment suggest that vertical vibration can accelerate rat lumbar vertebra facet joint degeneration, and it may cause low back pain. |
