To Investigate The Effect Of Tranexamic Acid On Blood Loss In Primary Unilateral Total Hip Replacement Surgery

Objective :To investigate the difference about blood loss of tranexamic in different use in primary total hip replacement surgery by contral experiment. Methods :From March 2014 to February 2016, 84 patients which meet the ondition of total hip arthroplasty are selected, 41 of them are male and 43 of them are female. Among them femoral neek fractrure ware 45 case, osteonecrosis ware 39 case,age between 51 to 83 years old and 68.5 is in average. All patients are divided into three groups at random, Group A incloud 26 case,group B incloud 30 case and group C incloud 28 case. For group A 100 ml normal saline was dripped half an hour before surgery and 100 ml normal saline was dripped after surgery. For group B tranexamic acid is diluted in 100 ml normal saline according to 20mg/kg dripped half an hour before surgery and 100 ml normal saline was dripped after surgery. For group C tranexamic acid diluted in 100 ml normal saline according to 10mg/kg was dripped half an hour before surgery, after surgery tranexamic acid diluted in 100 ml NS according to 10mg/kg was dripped.The following indicators such as age, weight, height, visib blood loss, hidden blood loss and coagulation index etc ware recorded and statistics. After operation symptom of deepvein thrombosis(DVT) and pulmonary thrombosis ware observered and be confirmed by color Doppler ultrasono graphy and CT angiography. Result :The difference of patients’ weight, height, age and gender were not statistically significant(P>0.05). There was no statistically significant in preoperative hemoglobin and blood coagulation indexes(P>0.05). The visib blood loss of group A, group B and group C was(288.37±25.94ml),(271.13±32.94ml),(190.49±35.01ml), Compared the intraoperative blood loss between ecah group the difference don’t have many significant differences(P>0.05). The hidden blood loss of group A group B and group C was(221.15±59.64ml),(183.19±63.35ml),(151.73±46.31ml), the difference between each group is significant(P<0.05),The total blood losing of group A group B and group C was(495.17±59.82ml),(448.86±76.06ml),(360.53±81.33ml), the difference between the three groups was statistically significant(P<0.05). Conclusion:1. Intravenous use of tranexamic acid can significantly reduce blood loss after primary unilateral total hip replacement. Tranexamic acid; and there is no increase in the risk of venous thrombus embolism, pneumonia, wound infection.2. In the condition of the same dose of unit weight is divided into preoperative 1/2, postoperative 1/2 twice intravenous use is more efficient than preoperative once and for all tranexamic acid used.
