Objective To discuss the clinical significance of BCM in the treatment of brainstem cavernous malformation by using the non operative indications, timing of operation and the use of functional neural navigation and intraoperative electrophysiological techniques.Methods A retrospective analysis of the First Affiliated Hospital of Fujian Medical University 2009 January 2015 December BCMs 40 patients, among them without surgical patients 8, routine surgical cases in 14 cases, 18 patients with functional neuronavigation and intraoperative electrical physiological monitoring technology guide the surgical patients, analysis focuses on analyzing to fulfill the non operative indications, surgical timing and functional neuronavigation and intraoperative electrophysiological techniques and conventional surgical treatment of brainstem cavernous hemangioma treatment compared to understand whether there is absolute superiority.Results After 16-32 months(mean 22 months) follow-up, 8 patients with non operative patients were found to have no re bleeding. Until the last follow-up, 6 patients did not have clinical signs and symptoms, KPS score of 100 points; 2 patients with mild symptoms and signs, KPS score 90 points. A summary of our data, more than 70% surgery patients was 1 months after surgery in the treatment of bleeding, but positive, this part of the brain stem cavernous malformation patients can obtain satisfactory results; after comparison, patients with brainstem cavernous malformation surgery using electrophysiological monitoring of neuronavigation and intraoperative cranial after nerve injury and aggravation of symptoms was significantly lower, and the signs and symptoms of cranial nerve injury is lighter, postoperative follow-up KPS score comparison is higher, the quality of life of patients has been improved to a certain extent, using the Fisher exact probability test of multimodal group(combined electrophysiological functional neuronavigation and intraoperative monitoring) postoperative improvement rate relative to the non multimodal operation group is statistically significant, the P<0.05, has statistical significance.Conclusion The selection of appropriate surgical and non surgical disease and reasonable operation time, adapt to the application at present the new functional neuronavigation and intraoperative electric physiological monitoring techniques guiding surgical treatment is to fulfill the treatment success is important assure.
