The Time And Therapeutic Effect Of Locking Plate’s Surgery Cure For Neer III-IV Type Fracture Of Proximal Humerus

Objective: To investigate the time and therapeutic effect of locking plate’s surgery cure for Neer type III-IV proximal humerus fractures.Methods: Retrospective analysis the clinical data of the patients of locking plate’s surgery cure for Neer type III-IV proximal humerus fractures from January 2010 to August 2015 in our hospital who was trested with operation,51 patients were included.Comparative analysis of two groups of intraoperative blood loss, fracture clinical healing time, elbow function activity,complications and so on. The cases of injury:24 cases of traffic accident injury,27 cases of falls.The clinical data of all patients were improved and regular follow-up was completed. According to the different operation time of injury,the three groups were divided into groups:the group A was 23 cases of 24 h patients, the group B was 28 cases of 4-7 days after the injury.All patients before surgery were to improve the shoulder positive lateral position,shoulder joint CT and other imaging examinations to determine the degree of fracture block and the degree of fracture classification,in which Neer type III type was 23 cases,and IV type 28 was cases.Preoperative examination perfect no contraindication,and were given plate fixation.Comparative analysis of two groups of intraoperative blood loss,operation time and neer score.Results: Group A’s operation time was shorter than group B,and the difference was statistically significant;The patients’ bleeding loss in group A was less than group B,the difference is statistically significant; the neer score after operation 3 months of group A was better than that of group B,the difference is statistically significant.The differences of two groups’ s neer score excellent rate after operation 3 months had no statistical significance. Conclusion: The elderly patients of proximal humeral Neer type III-IV fractures after injury should be early surgical treatment,within 24 hours after injury day surgery is the best;if not in 24 hours underwent surgical treatment should be avoid injury after inflammation and swelling of the soft tissue peak,choose in postoperative 4-7 days operation treatment can obtain satisfactory surgical results.
