| [purpose]To analyse the clinical effects of modified cervical expansive laminoplasty and explore its effects on axial symptoms in treatment of cervical spondylotic myelopathy.[Methods]A total of 17 patients from March 2012 to Apri 2015 in Fujian Meical University Union Hospital.8 cervical spondylotic myelopathy patients who underwent modified cervical expansive laminoplasty(group A) and 9 cervical spondylotic myelopathy patients who underwent traditional cervical expansive laminoplasty(group B) were included in this study.Compare the clinical effects between modified cervical expansive laminoplasty and traditional cervical expansive laminoplasty,and observe the effects on axial symptoms after the operations.[Results]There were no significant differences found in the gender,age,JOA(Japanese Orthopaedic Association) scores before the operations,disease time and the time of follow-up study.And also no significant differences in surgical time and the blood loss during and after operations.The JOA scores of 1、6、12 months after operations and last exam time are 12.63±0.52、13.38±0.52、13.75±0.46、14.13±0.35 in group A,and 12.78±0.67、13.44±0.53、13.78±0.44、14.00±0.50 in group B,and there was no significant difference between them(P>0.05).The incidence rate of axial symptoms were 37.5% in group A and 55.6% in group B.and also the recovery time of axial symptoms is 93±25 days in group A which is shorter than 192±107 days in group B(P<0.05).[Conclusion]modified cervical expansive laminoplasty and traditional cervical expansive laminoplasty have the same clinical effects in treatment of cervical spondylotic myelopathy,and modified cervical expansive laminoplasty can reduce the incidence and degree of postoperative axial symptoms,and also reduce the time needed for pain relief. |
