?Background?With the improvement of color Doppler ultrasonography technology, thyroid fine needle biopsy(fine needle aspiration biopsy, FNAB) technology is widely used, and the health consciousness of people increase,the morbidity of thyroid cancer is increasing significantly. Because most of differentiatiaed thyroid cancer are "inert cancer", so it arouses the controversy for the thyroid diagnosis scheme choices. Recent studies have shown that BRAF V600 E mutations,Galectin-3 protein expression are correlated with lymph node metastasis of thyroid carcinoma, capsule extension, and recurrence.?Objective?To analysis the correlation about BRAF V600 E mutation, Galectin-3 protein expression and lymph node metastasis and explore the diagnostic value with BRAF V600 E mutation, Galectin-3 protein expression for thyroid cancer.?Methods? 278 patients who accepted surgical treatment in Fujian Shengli hospital and were diagnosed differentiatiated thyroid cancer by pathology collected from 2014.01 to 2015.01.Inclusion criteria: all pathological cases refer to the WHO classification and diagnostic criteria of thyroid tumor, and pathological diagnosis by two experienced attending and above levels of the physician to diagnose;All the patient had no history of thyroid surgery, radiotherapy or other history of malignancy.Thyroid cancer of the Gal- 3 protein expression was detected by immunohistochemical Envision ldpe-g-nvp;For all cases were detected with polymerase chain reaction(PCR) and DNA sequencing method BRAF V600 E gene mutation.?Result?1. The BRAF gene mutation or Galectin-3 has no correlation with the lymphatic metastasis of the differentiatiated thyroid cancer(P > 0.05) 2. The BRAF gene mutation is associated with multifocality(P < 0.05).3. The Galectin-3 protein expression is associated with cancer cells invade surrounding tissue(P < 0.05).4. The BRAF gene mutation in thyroid carcinoma has collection with Galectin-3 protein expression(P < 0.05).5.Meanwhile,the single factor analysis for lymph node metastases: the age(45 years old or more), and extrathyroidal invasion, multifocality are associated with the lymph node metastases(P < 0.05);Multivariate analysis: The age(45 years old or more) and the concomitant hashimoto thyroiditis are the protection factor for the lymph node metastases(P < 0.05); The multifocality is a risk factor for the lymph node metastases(P < 0.05).2.?Conclusion?1. The BRAF gene mutation and Galectin-3 have no correlation with differentiated thyroid cancer lymph node metastases, but when the BRAF gene mutated, the rate of lymph node metastasis increases significantly, it means prophylactic lymph node cleaning may be performed.2. When the BRAF gene mutation, Galectin-3 protein expression strength enhanced obviously, thyroid cancer develops multiple central lesions may increase, it means total thyroidectomy may be performed.3. The Galectin-3 protein strong expression positive, the ability of thyroid cancer invade into the surrounding tissue will be enhanced, it prompts excision scope may be more complete. It should do preventive cleaning lymph node in central region, if the patient is less than 45 years old with thyroid cancer;If the patient is more than 45 years old, or combines with the hashimoto, it will reduce the risk of differentiatiated thyroid cancer's lymph node metastasis significantly.When the differentiatiated thyroid cancer's has multiple lesions, it significantly higher risk of the lymph node metastases than the single cancer.
