Plasma Oxidized High-density Lipoprotein And Glycated Apolipoprotein A-Ⅰ Levels In Patients With ST-segment Elevation Myocardial Infarction And Their Clinical Significance

Objective:To investigate the plasma levels of oxidized high density lipoprotein (ox-HDL)and glycated apolipoprotein A-I (gly-ApoA-I)of the acute ST-segment elevation myocardial infarction(STEMI) patients and the correlation with C reactive protein (CRP), brain natriuretic peptide (BNP), cardiac troponin I(cTNI) and left ventricular ejection fractions(LVEF).Methods:55 patients with acute STEMI divided into stress hyperglycemia (SH) group of 28 cases and no stress hyperglycemia (no-SH) group of 27 cases or high thrombus burden (HTB) group of 29 cases and low thrombus burden (LTB) group of 26 cases. All patients received primary percutaneous coronary intervention(PCI) treatment and standardized drug treatment. Meanwhile,48 healthy volunteers were collected as the control group. On admission and 7 days, plasma ox-HDL levels were measured by enzyme linked immunosorbent assay (ELISA) and gly-ApoA-I were detected by Western blot. We also calculated ox-HDL/HDL-C and relative intensity of ApoA-I glycation.Results:The plasma levels of ox-HDL and gly-ApoA-I on admission were higher than the control group and the levels of 7 days. The levels of ox-HDL and gly-ApoA-I in SH group were higher than no-SH group. HTB group also had higher results than LTB group. Furthermore, ox-HDL and ox-HDL/HDL-C levels were found to be positively correlated with CRP, BNP and cTNI. In addition,ox-HDL/HDL-C levels were also negatively correlated with left ventricular ejection fractions (LVEF) in STEMI patients.Conclusions:The results indicate that circulating ox-HDL and gly-ApoA-I maybe very important in pathogenesis of STEMI. ox-HDL and ox-HDL/HDL-C might be predictors of inflammation after STEMI,infarct expansion and prognosis. In addition, ox-HDL/HDL-C has better predictive value for prognosis and should be a potentially novel biomarker for patients with STEMI.
