Objects:Kupffer cells antigen presenting function plays an important role in the pathogenesis of autoimmune liver diseases( ALD). In this paper,we discuss the expression of Kupffer cells antigen presenting molecules in autoimmune liver diseases.First of all, through the CD40/CD68, CD80/CD68, HLA-DR/CD68 immunofluorescence staining, HLA-DR, CD80 and CD40 were clearly expressed in the liver Kupffer cells,And then CD40 and HLA-DR were compared by immunohistochemical staining,in order to explore the changes of CD40 and HLA-DR, and to explore whether there is a correlation between them.Secondly analyzing the relationship between CD40, HLA-DR and liver function, blood coagulation function and other laboratory results,reveal the role of Kupffer cells antigen presenting molecules in the pathogenesis of autoimmune liver disease.Methods:(1)Patients who were admitted to the Tianjin General Hospital from January 2010 to June 2015 were treated with liver biopsy, were divided into autoimmune hepatitis(AIH) group, primary biliary cholangitis(PBC) group, primary biliary cirrhosis and autoimmune hepatitis overlapsyndrome(PBC-AIH OS) group.Alcoholic liver disease, fatty liver, chronic viral hepatitis and other non autoimmune liver disease patients as a control group.The blood biochemical and coagulation function of the selected patients were recorded.(2) CD40/CD68, CD80/CD68 and HLA-DR/CD68 wereusedby immunofluorescence staining.(3)Immunohistochemical staining of CD40 and HLA-DR were performed,random selection of 10 high-power mirror field of vision(400 times),then compare the differences with variance analysis(4) Compare the correlation of CD40 and HLA-DR.(5)Compare the correlation of CD40, HLA-DR and the liver function, blood coagulation function.Result(1)Immunofluorescence double staining showed that CD40, CD80, HLA-DR were expressed in the liver CD68+kupffer cells.(2) The number of the positive cells in the autoimmune liver disease group was lower than that in the control group.The positive product is brown granular or brown stain, mainly located in the cell membrane, little part is located in the cytoplasm. The results showed that there wasstatistical significance(P = 0.04, P < 0.05)among the three groups.There was no significant difference between the AIH group and the PBC group(P=0.428, P>0.05).There was significant difference between AIH and NALD(P=0.02, P<0.05).(3) HLA-DR in autoimmune hepatitis group, the positive cells was lower than the control group, the staining positive products were brown particles, mainly located in the cell membrane.The absolute value of HLA-DR positive Kupffer cell count was calculated and the results showed that the 3 groups had statistical significance(P=0.015, P<0.05),There was no statistically significant between AIH and NALD(P=0.017, P<0.05).(4) CD40 and HLA-DR showed a positive correlation.(5) CD40, HLA-DR showed correlation with ALT/AST in AIH.Conclusion:(1) CD40,CD80 and HLA-DRwere located in CD68+kupffer cells, ?and changes in antigen presentation molecules may cause changes in the function of antigen presentation.(2)CD40 and HLA-DR in patients with autoimmune liver disease was less than that of non autoimmune liver disease, and CD40, HLA-DR and liver function damage is correlated. It suggested that autoimmune liver disease may be weakened by Kupffer cell antigen presenting function, which is one of the causes of liver damage in autoimmune liver diseases.
