OBJECTIVE With the improvement of life standardsand the change of dietary habits, the incidence ofacute pancreatitis(AP) had increased, while the etiological factors of AP hadalso changed during the past twenty years. Biliary tract disease have been the most common cause of AP, however, the rising incidence of the hyperlipidemic acute pancreatitis(HLAP)was closely associated with the incidence of obesity. To investigate etiology as well as clinical profile of AP in Xi'an region, so as to provide basis for clinical prevention and remedy, we analyzed the differences of the related clinical indicators of AP among different etiologic factors.METHODS We enrolled 409 patients who were diagnosed as AP between December 2013 and December 2015 from four tertiary hospitals in Xi'an into a multi-center, retrospective study. All the subjects were divided into five groups according to the etiologic factors. The constituent ratio and changes of different factors, and the clinical indicators of different factors in different age and gender were analyzed. The proportion of AP patients combined with nonalcoholic fatty liver diseases was analyzed at the same time.RESULTS In409 cases of AP, 219patients(53.6%) were diagnosed with acute gallstone pancreatitis,68 patients(16.6%) were HLAP, 56 patients(13.7%) were alcoholic pancreatitis, 39patients(9.5%)were dietary factors-related pancreatitis, and 27cases(6.6%) were other causes related acute pancreatiti. 159 were diagnosed as acute pancreatitis combine with nonalcoholic fatty liver, and 113 cases(71%) were obesity among these 159 cases. Age difference was found in different etiologic factors(P<0.05), and most patients of the gallstone pancreatitis were female(P<0.05).The serum aminotransferase, cholesterol.low density lipoprotein levelsand triglyceride levels, blood sugar level, body mass index(BMI) all had differences among these etiologic factors.(P<0.05),whereas there were no statistical differences of white blood cells, blood calcium levels among them were found(P>0.05).CONCLUSIONSBiliarytract disease was still the main cause of AP, while the incidence of HLAP had been gradually rising in recent years. Age, gender and clinical indicatorshad differences among acute pancreatitis caused by different etiologic factors. Obesity, nonalcoholic fatty liver disease and pathoglycemia closely interrelated with and influenced the development, prognosis of hyperlipidemic acute pancreatitis.
