Objective: The clincial characteristics and data of patients with posttraumatic acute diffuse brain swelling(PADBS) were retrospective analyzed in our hospital. The related factors for posttraumatic acute diffuse brain swelling(PADBS) were analyzed. And the independent risk factor for PADBS were screened by logistic regrestion analysis.Methods:1.Two hundred and forty four case of brain traumatic injury patients were retrospectively analyzed in our hospital. And of the screened 244 subjects, 98 were included in our prestnt study for their completed data and clincial information. 2. For the included 98 brain trammatic injury patients, 22 were found have the posttraumatic acute diffuse brain swelling(PADBS) and other 76 cases were not found the PADBS history. The clinical data and information such as age, gender, time form injury to treatment and et al were recorded and compared between the two groups by chi-square or student-t test to evaluate the statistical difference. 3. The factors with statistical difference between the two groups such as the treatment of more than 3 hours, aspiration, hypotension and primary brain stem injury were further bring into the logistic regression equation and to screen the independent risk factors for acute diffuse brain swelling.Results: 1. Finally, two hundred and forty four paties were screened in hour hospital database. And 98 subjects were included our present with complete clinical information and data. Of the 98 cases, 22 patients were found PADBS and other 77 were not which were considered of cases and control gorup respectively. 2. The potential risk factors were analyzed by chi-square test or student-t test to compared the difference between the two grops. The results showed that there were no statistical difference in the aspects of age(P>0.05), gender(P>0.05),history of diabetes(P>0.05), history of hypertension(P>0.05) multiple truma injury(P>0.05) and skull base fracture(P>0.05). But significant statistical difference were found in the aspects of time form injury to treatment(P<0.05), pulmonary aspiration(P<0.05), low blood pressure shock(P<0.05) and primary brain stem injury(P<0.05). 3. The factors with statistical difference between the two groups such as the treatment of more than 3 hours, aspiration, hypotension and primary brain stem injury were further bring into the logistic regression equation and to screen the independent risk factors for acute diffuse brain swelling. The results showed that long period from injury to treatment(OR=2.36,P=0.018), low blood pressure shock(OR=3.23,P=0.031) and primary brain stem injury(OR=3.69,P=0.025) were independent risk factors for PADBS.Conclusion: Acute diffuse brain swelling(acute diffuse brain swelling posttraumatic, PADBS) in patients with traumatic brain injury is common clinically with high mortality rate. Patients with delayed treatment, low blood pressure, and primary brain stem injury. Are more like to develop acute diffuse brain swelling. Therefore, for PADBS patients with above characteristics shoud be paid much attention to decrease their mortality rate.
