| Objective: To detect serum intact parathyroid hormone(iPTH)and retinol-binding protein(RBP)levels in maintenance hemodialyis(MHD)with type 2 diabetes mellitus(T2DM)or non T2 DM subjects and investigate the relationships between serum iPTH levels and RBP levels.Methods: A total of 77 patients were enrolled in this study that all of them have been diagnosed end-stage renal disease.All patients were maintained hemodialysis.77 patients were divided into diabetic nephropathy(DN,n=33)group and non diabetic nephropathy(NDN,n=44)group.All of the subjects were detected in serum fasting blood glucose(FBG),creatinine(Cr),blood urea nitrogen(BUN),β2microglobulin(β2MG),uric acid(UA),serum alkaline phosphatase(AKP),serum calcium(Ca)and phosphorus(P)and the level of blood lipid.Enzyme immunoassay was used to detect the level of RBP.iPTH was detected by chemiluminescence method.Results: 1.There was no significant difference of RBP level between DN group [98.9(55.8150)mg/L] and NDN group [87.8(53.3150)mg/L](P>0.05).2.Serum iPTH level in DN group [207(43688)ng/L] was significantly lower than NDN group [285.4(69899)ng/L](P<0.05).Serum FBG level(8.87±3.15)mmol/L in DN group was significantly higher than NDN group(4.94±1.45)mmol/L(P<0.05).3.Correlation analysis showed that the serum iPTH level was positively correlated with Cr、β2MG、RBP and P.Correlation coefficient r was 0.628、0.659、0.621 and 0.542 respectively.(P<0.05).Serum iPTH level was negatively correlated with Ca and FBG,Correlation coefficient r was-0.516 and-0.554(P<0.05).4.The multiple stepwise regression analysis showed that P、RBP and β2MG is risk factors to maintenance hemodialysis of diabetic nephropathy patients(P < 0.05).Conclusion: 1.Serum iPTH was significantly lower in DN group than that in NDN.2.RBP levels in DN group had no significant difference compared with NDN group.3.Serum iPTH in DN group are closely related with levels of FBG、RBP、P 、Ca 、Cr and β2MG.4.Clinical monitoring and interventing iPTH in patients with diabetic nephropathy that having maintenance he maintenance hemodialyis modialyis can help understand the occurrence and progressions of diabetic nephropathy and have effective methods to evaluate the complications of diabetic nephropathy. |
