Purpose:To objectively evaluate the clinical effect, safety and comparative advantages to western medicine of the treatment of blood vacuity and congealing cold diabetes with peripheral neuropathy by modified Dangguisini decoction, and provides reference for the treatment of diabetes with peripheral neuropathy by traditional Chinese medicine through clinical observation.Method:All the 50 cases in this study were selected from the Department of Endocrinology of Hainan Hospital of TCM,and had been divided into 2 groups with 25 cases in each group by a random number table method according to the diagnostic classification criterion.Patients in both groups received conventional glucose-lowering therapy. There were no statistically significant differences between the two groups in blood glucose control.Treatment group accepted foot bath with modified Dangguisini decoction(angelica sinensis, cassia twig, asarum, white paeony root,Chinese date, glycyrrhiza, tetrapanax papyriferus, astragalus, spatholobus suberectus dunn)once per day and 30min for each time;while the control group was treated with Mecobalamin Tablets(Mi Ke Bao),0.5mg each time,3 times per day, oral administration.2 months later, compared the TCM symptoms, hemorheology,Toronto clinical scores and electromyography(EMG)of the patients before and after the treatment.Result:Following completion of treatment, the therapeutic outcomes were subject to statistical analysis using SPSS13.0 software.Comparisons between the two groups are as follows:1.TCM symptoms:Improvement was observed in both groups; But the total effective rate of the treatment group was higher than that of the control group, the treatment group was 92%,and the control group was 56%,with statistically significant difference(P<0.05);2.Hemorheologic indicators:Greater improvements in whole blood viscosity and plasma viscosity were observed in the treatment group than in the control group, with statistically significant differences(P<0.01).3.TCSS score:Both the treatment and control groups presented decreases in TCSS score from pre-treatment to post-treatment(P<0.01),and the treatment group presented a more significant trend towards decreasing as compared to the control group, with very statistically significant difference(P<0.01);4.EMG:The post-treatment conduction velocity of median nerve and peroneal nerve presented statistically significant differences from pre-treatment in both groups(P<0.01 in the treatment and control groups, respectively);comparison between groups indicated greater increase in post-treatment nerve conduction velocity in the treatment group, with very statistically significant difference(P<0.01);5.Safety:No abnormality in hepatic or renal function was observed in either group before or after treatment, and no adverse reactions possibly related to the study treatment were observed. Conclusion:It’s been confirmed that the modified Dangguisini decoction used as external washing can improve the clinical symptoms of blood vacuity and congealing cold type diabetes with peripheral neuropathy, accelerate the neural conduction velocity, improve the indicators of hemorheology, and it’s obviously better than purely treated with western medicine and has definite clinical effect,so it’s worth spreading use.
