Objective:In this study,CBCT was used to measure and analyze the upper airway dimension and hyoid bone position in skeletal Class I patients,then to discuss the relationship between the upper airway dimension and hyoid bone position with craniomaxillofacial.The revealed relationship will provide some reference for the study of craniofacial growth and orthodontic clinical diagnosis and treatment.Method:CBCT images of 60 adult patients with skeletal class I were selected according to MP-SN angle in the CBCT database of Shandong University,which was 18-35 years old and local residents of Shandong Province.Upper airway volume,cross-sectional area,sagittal diameter,transverse diameter and the position of the hyoid bone were measured by Dolphin software.All the measured results were analyzed by SPSS22.0 statistical software.A single-factor analysis of variance was performed between the same gender groups with different vertical skeletal groups.T-test was used for comparison of the gender-related differences in the same vertical skeletal pattern group.The airway measurement data were correlated with ANB angle,MP-SN angle and hyoid bone position,Pearson correlation analysis was performed between the measurement indexes.Test level bilateral α=0.05.Result:1.Differences of upper airway and hyoid bone position in different vertical skeletal pattern groupsThere were no statistical significance(P>0.05)except the sagittal diameter of EB in line distance;The cross sectional area and volume of the upper airway become more and more lager from high angle group to low angle group,but there were no statistical significance(P>0.05)except V,VOP,VPP,VGP,SUT,SET.There was no significant difference in the H-PS between the different groups.The H-VPS of low angle group is the largest in the different three groups,which was statistically significant(P<0.05).2.Gender-related differences in adult patients with the same vertical skeletal patternsThe airway volume and cross-sectional area of men’s upper airway were all higher than women’s,but there were no statistical significance(P>0.05)except VGP(in the high angle group and the low angle group),SET,SMIN.The sagittal diameter and transverse diameter of the men’s upper airway in the restricted plane were slightly larger than women’s,but there was no significant difference(P>0.05)except the LET.The levels of H-PS and H-vps were significantly higher in the male group than in the female group(P<0.05).3.The correlation analysis betewen MP-SN and ANB with upper airway and hyoid bone positionMP-SN was negatively correlated to measurements of the upper airway and hyoid bone position,while ANB had no correlation with them.There was a statistically significant correlation between the upper airway morphometric index and the hyoid bone position measurement.Conclusion:1.There was a significant differences with oropharyngeal airway in different vetical patterns group,but there was no significant difference between nasopharyngeal and laryngeal airway.With the increase of mandibular plane angle,the dimension of oropharyngeal airway more smaller and the horizontal position of Hyoid bone more backwards;2.There were gender differences in upper airway morphology and hyoid bone position in adult skeletal class I patients with the same vetical pattern.The oropharyngeal airway of male was larger than that of female,and the men’s hyoid bone position is more front and lower than women’s;3.The vertical pattern has a significant effect on the upper airway morphology and hyoid bone position,and the upper airway morphology and hyoid bone position were closely related;4.In the preparation of orthodontic treatment plan,we need to take the gender factor into account and in order to avoid the occurrence of iatrogenic OSAHS we should pay attention to vertical controlling in the orthodontic treatment process.
