ObjectiveStudy on metabolic syndrome(MS)of phlegm syndrome of different disease and miscellaneous characteristics,analysis of the correlation between phlegm symptom factors and physicochemical indexes,to explore the relationship between the MS phlegm syndrome of different disease location disease and miscellaneous,and IGF2BP2,FTO gene polymorphism,to reveal the common and individual characteristics of TCM syndromes in different syndromes of MS phlegm syndrome,to clarify the genetic basis of the syndrome formation,to enrich the theory of etiology and pathogenesis of MS phlegm syndrome,and to provide basis for the treatment of MS syndrome differentiation.Methods1.According to 2015 IDF diagnostic criteria,314 cases of MS patients with phlegrm syndrome were divided into 4 different MS phlegm syndrome.18 cases of group A(central obesity,dyslipidemia and hypertension),66 cases of group B(central obesity,abnormal lipid metabolism,abnormal glucose metabolism,78 cases of group C(central obesity,abnormal glucose metabolism,hypertension),152 cases of group D(central obesity,abnormal lipid metabolism,abnormal glucose metabolism,hypertension).Using the method of syndrome differentiation with clinical data and other information to analyze the distribution of syndrome elements and physical and chemical indexes.2.Pearson and Spearman correlation analysis was used to analyze the correlation between the phlegm syndrome integral and the physical and chemical indexes of MS phlegm syndrome.3.SNPscanTM multiple SNP typing and TaqMan-MGB probe were used to detect the rs9939609 genotype of rs4402960 and FTO of IGF2BP2.Results1.Distribution of TCM syndromes in different syndromes of MS phlegm syndrome(1)Each group of phlegm syndrome score from high to low in turn is:D group>C group>B group>A group,D group compared with the other 3 groups,the difference was statistically significant(P<0.05).(2)The ratio of liver,spleen and kidney in common diseases was higher than that in lung and heart.The specific frequency of liver,spleen and kidney in the 4 groups were:group A:spleen>liver>kidney,B group,C group,D group,liver>spleen>kidney.The frequency of kidney in group A was lower than that in the other three groups,the difference was statistically significant(P<0.01).(3)The real common clinical manifestations of each group were dampness and heat,qi stagnation,blood stasis,4 groups of real venereal frequency from high to low:wet>heat>qi stagnation>blood stasis.The frequency of solid disease in group B was higher than that in the other three groups,the difference was statistically significant(P<0.01).The frequency of chronic obstructive pneumonia in group A was lower than that in the other three groups(P<0.05).(4)The common virtual diseases of all groups were Qi deficiency,yin deficiency,Yang deficiency,blood deficiency.4 groups of virtual disease frequency from high to low:group A:yin deficiency?Yang deficiency>Qi deficiency>blood deficiency;B group,D group,yin deficiency>Yang deficiency>Qi deficiency>blood deficiency;group C:yin deficiency>Qi deficiency>Yang deficiency>blood deficiency.2.Correlation between phlegm syndrome and physical and chemical indexes in different syndromes of MS phlegm syndrome(1)Compared with A group,B group,SBP,DBP,FPG,UA decreased significantly,HOMA-IR increased significantly,the difference was statistically significant(P<0.05);C group of FPG,LDL-C and HOMA-IR were significantly increased,UA decreased significantly,the difference was statistically significant(P<0.05);group D,FPG TC,TG,HOMA-IR,UA were significantly increased,the difference was statistically significant(P<0.05).Compared with B group,C group,BMI TG,UA SBP,DBP was significantly decreased,and LDL-C increased significantly,the difference was statistically significant(P<0.05);D group of BMI,SBP,DBP,TC were significantly increased,the difference was statistically significant(P<0.05).Compared with C group,DBP,FPG,TC,TG,LDL-C,UA were significantly increased in the D group,the difference was statistically significant(P<0.05).(2)The positive correlation with phlegm syndrome score was AST,and the difference was statistically significant(P<0.05).(3)The positive correlation degree between C group and phlegm syndrome score from high to low was:DBP,TG,FPG,the difference was statistically significant(P<0.05).(4)The positive correlation degree between D group and phlegm syndrome score from high to low was:BMI,SBP,UA,the difference was statistically significant(P<0.05).3.Correlation between rs4402960 syndrome and IGF2BP2 gene polymorphism in patients with MS phlegm syndrome(1)The distribution of genotypes in each group was in accordance with Hardy-Weinberg genetic balance.(2)There was no significant difference in genotype and allele distribution in each group(P>0.05).(3)The frequency of lung in group B was significantly higher than that in non-lung(P<0.01);The frequency of liver in GT group was significantly lower than that in non-liver group,the difference was statistically significant(P<0.01);The frequency of GT in patients with solid disease was significantly lower than that in non-blood stasis(P<0.01).4.Correlation between rs9939609 syndrome and FTO gene polymorphism in patients with MS phlegm syndrome(1)The distribution of genotypes in each group was in accordance with Hardy-Weinberg genetic balance;(2)There was no significant difference in genotype and allele distribution in each group(P>0.05).(3)The frequency of AA in group B was significantly lower than that in non-wet group,the difference was statistically significant(P<0.05);The frequency of deficiency of TA in group B was significantly higher than that in non-qi deficiency,the difference was statistically significant(P<0.01);The frequency of D in solid disease was significantly lower than that in non-wet group(P<0.01).Conclusions1.The distribution of syndromes in different syndromes of phlegm syndrome showed the characteristics of deficiency and reality.2.There was a correlation between phlegm syndrome and clinical physical and chemical indexes in different syndromes of MS phlegm syndrome.3.The formation of TCM syndromes in different syndromes of MS phlegm syndrome is closely related to IGF2BP2 and FTO gene polymorphism.
