Objective:to analyze clinical characteristics,treatment regimens and prognostic factors of 122 patients with Hodgkin’s lymphoma who newly diagonosed in our hospital,and to evaluate the curative effects as well as toxicity of the treatments.Methods:the clinical data of all patients with Hodgkin’s lymphoma who underwent initial treatments in our hospital.from August 2007 to July 2016 were collected.Results:the median follow-up time of all patients was 38 months(2~112months),the median age of all patients was 28 years old(from 3 to 85 years old).There were 91 male patients and 31 female patients,59 of whom suffered from stage Ⅰ and ⅡHodgkin’s lymphoma and 63 from stage Ⅲ and Ⅳ Hodgkin’s lymphoma,58 combined with B symptom and 18 with bulky mass.109 cases of patients underwent ABVD regiment chemotherapy alone,with the number of median cycles of 4(from 0.5 to 8 cycles),10 cases accepted ABVD+ BEACOPP-like regiments chemotherapy and 3 cases underwent ABVD chemotherapy combined with other regimens,48 cases were treated with chemotherapy alone and 74 with chemotherapy combined with radiotherapy,radiotherapy dose ranged from 9 to 44.8Gy and median radiotherapy dose was 30.6Gy.For the whole group,91 cases had complete(CR),26 cases had partial remission(PR)and 5 cases experienced disease progression(PD).5-year overall survival(OS)rate of all patients were 76.8%,5-year OS rate of all patients at early-stage and at advanced-stage were 86.7%and 70.6%(P=0.096),respectively,the difference being not statistically significant,5-year OS rate of patients suffering from stage Ⅰ and Ⅱ Hodgkin’s lymphoma as well as stage Ⅲ and Ⅳ were 83.5%and 70.3%(P=0.066),respectively,the difference being not statistically significant.7 of 59 patients suffering from stage Ⅰ and Ⅱ Hodgkin’s lymphoma died,including 1 case died of tumor progression,1 case of pulmonary infection,1 case of traffic accident and 4 cases of unknown cause,15 of 63 patients suffering from stage Ⅲ and Ⅳ Hodgkin’s lymphoma died,7 cases died of pulmonary infection,4 cases of tumor progression,1 case of intracranial infection,1 case of suicide and 2 cases of unknown causes.5-year OS rate of the group of early-stage with favorable factors and with unfavorable factors were 84.8%and 83.2%(P=0.934),respectively,the difference being not statistically significant,5-year OS rate of patients suffering from stage Ⅲ and ⅣHodgkin’s lymphoma classfied according to International prognostic scores into 0-1 points,2-4 points and>4 points were 89.2%,66.0%and 35.7%(P=0.128),respectively,the difference being not statistically significant.3 of 16 cases in early-stage with favorable factors of were treated by chemotherapy alone and 13 by chemotherapy combined radiotherapy,and both the dead cases were treated with chemotherapy alone,difference of 5-year OS rate being statistically significant(P<0.05).In all patients from stage Ⅲ to Ⅳ,52 cases were treated with ABVD regimens chemotherapy and 8 cases with ABVD+BEACOPP-like regimens chemotherapy,and 13 cases died,including 2 treated with ABVD+BEACOPP-like regimens and 11 cases with ABVD regimens,the two different regiments of 5-year OS rate being not statistically significant(P>0.05).Univariate analysis showed the age,β2-microglobulin and the ratio of lymphocyte to Monocyte count were prognostic factors for 5-year OS rate of all patients.Multivariate analysis showed the age,cycles of ABVD regiments and chemotherapy regiment were independent prognostic factors for 5-year OS rate of all patients,the age and level of albumin of advanced-stage HL were independent prognostic factors for 5-year OS rate.In terms of common toxicity effects of ABVD regimen,the incidence rates of grade two and three leukopenia,grade three and four neutropenia,grade one and two aminotransferase elevation were 61.5%(75/122),40.1%(49/122)and 46.7%(57/122).Conclusion:HL display a high incidence in young male patients,overall therapeutic effect in the treatment of Hodgkin’s lymphoma is good,but because the majority of patients failed to complete treatments,the overall survival rate decreased slightly.Radiotherapy can improve the 5-year OS rate of early-stage HL with favorable factors.Age,β2-microglobulin,the ratio of lymphocyte counts to mononuclear cell in peripheral blood as well as serum albumin may influence the prognosis of patients.
