Objective:To investigate the clinical related factors and prognosis of patients with secondary cerebral infarction due to carbon monoxide(Carbon monoxide,CO)poisoning,so as to provide reference for the treatment and prognosis evaluation of CO poisoning secondary cerebral infarction patientsMethods:From January 2011 to September 2015 the 151 patients of with acute carbon monoxide poisoning at the Afiliated Hospital of Yanbian University were enrolled in the study.Demographic and clinical information was collected.According to the patient's head imaging,ninetieth day modified Rankin score in patients with secondary diagnosed cerebral infarction of carbon monoxide poisoning(Modified Rankin Scale,mRS),Number days of latent period by multi factor Logistic regression analysis model to clear CO poisoning related clinical factors and prognosis of independent risk factors.Results:1.Carbon monoxide poisoning in the secondary cerebral infarction group and the non-secondary cerebral infarction group,Multivariate logistic regression analysis model:after adjusting univariate analysis of risk factors associated with new onset cerebral infarction(Diabetes,hypertension,smoking,alcohol consumption,body mass index,fibrinogen determination),Diabetes(OR 12.673,95%CI 4.100-39.175,P<0.001),hypertension(OR 3.315,95%CI 1.247-8.811,P=0.016),alcohol consumption(OR 3.272,95%CI 1.083-9.885,P=0.036)were independent risk factors of cerebral infarction by carbon monoxide poisoning.2.Carbon monoxide poisoning in patients with good prognosis of cerebral infarction and good prognosis in the group with multiple factors logistic regression analysis model:after adjusting univariate analysis of risk factors in patients with poor prognosis(alcohol consumption,total cholesterol concentration,the use of edaravone,assisted ventilation,fibrinogen determination),alcohol consumption(OR 45.454,95%Cl 1.190-9.434,P=0.022)was independent risk factors of cerebral infarction prognosis of carbon monoxide poisoning.The use of edaravone(OR 0.274,95%CI 0.107-0.701,P=0.007)were the protective factors for cerebral infarction prognosis of carbon monoxide poisoning.Assisted ventilation group and non assisted ventilation of the admission NIHSS score(National Institutes of Health Stroke scale,NIHSS),ninetieth day of NIHSS score,NIHSS score decreased numerical analysis of variance:the Ninetieth day of NIHSS score(F=4.589,P=0.034),the NIHSS decreased(F=8.009,P=0.006)difference was statistically significant.3.Multiple factor Logistic regression analysis model of delayed encephalopathy of carbon monoxide poisoning in 2-30 days and 30-60 days:after adjusting univariate analysis of the risk factors for prolonged false recovery[Diabetes,hypertension,smoking,alcohol,low density lipoprotein cholesterol(Low density lipoprotein cholesterol,LDL),high density lipoprotein cholesterol concentration(High density lipoprotein cholesterol,HDL)],Diabetes(OR 59.087,95%Cl 5.826-7.054,P=0.001),hypertension(OR 19.040,95%CI 2.714-133.576,P=0.003),smoking(OR 10.640,95%CI.748-64.760,P=0.010),alcohol(OR 29.429,95%CI 3.666-236.231,P=0.001)were independent risk factors of prolonged latent phase time.Conclusion:The influence of risk factors,latent phase of carbon monoxide poisoning prognosis of patients with cerebral infarction and delayed encephalopathy such as diabetes,hypertension,drinking,early assessment of carbon monoxide poisoning patients,the use of edaravone on ventilation positive intervention may improve the prognosis.
