Objective:To explore the influence of different degree of White matter ischemic lesions(WMIL)on the cognitive function of patients and the characteristics of cognitive dysfunction;Detect the expression level of Anti-Myelin oligodendrocyte glycoprotein(MOG)antibody and intercellular adhesion molecule-1(ICAM-1)in peripheral blood of different degree WMIL patients;Analysis the related factors of WMIL,and the correlation between WMIL and anti-MOG antibody,ICAM-1.Methods:From November 1,2015 to December 31,2016 enrolled patients of Su Zhou Hospital Affiliated to Anhui Medical Uniwersity,and 115 cases of WMIL were confirmed by cerebral Magnetic resonance imaging(MRI).21 cases were enrolled from healthy physical examination person,and no white matter lseions confirmed by cerebral MRI.Based on Fazekas criteria,WMIL group of patients were dividedinto light,medium and heavy groups,mild group 1-4 points,moderate group 5-8 points,severe group 9-12 points.According to different part of lesios,WMIL group were divided into Periwentricular white matter lesions(PWML)group and Deep white matter lesions(DWML)group.The congitive performance was assessed by using Montreal Cognitive Assessment(Mo CA).Anti-MOG antibodies and ICAM-1 level were examined by using ELISA.Results: 1.The Mo CA scale score of WMIL group was obviously lower than that of healthy controls(P<0.05);The Mo CA scale score of mild WMIL group had no difference between healthy controls(P>0.05);The Mo CA scale score ofmedium and heavy WMIL group was significantly lower than that of healthy controls(P < 0.05);The Mo CA scale score of mild,medium and heavy WMIL these three groups had differences(P < 0.05),and the correlation analysis showed that Mo CA scale scores were negatively correlated with Fazekas score(P < 0.05);Cognitive function of patients in DWML group were significantly decreased compared with the PWML group(P < 0.05).2.According to the analyze of associated risk factors,WMIL group and healthy controls had obvious difference in age,smoking history,blood lipids and homocysteine(P < 0.05).3.Peripheral anti-MOG antibody level in WMIL group was obviously higher than that of healthy controls(P < 0.05);The peripheral anti-MOG antibody levels of light,medium and heavy WMIL these three groups had differences(P < 0.05),correlation analysis showed that peripheral anti-MOG antibody level was positively correlated with Fazekas scores(P<0.05),and negatively correlated with Mo CA scale scores(P < 0.05).Peripheral anti-MOG antibody level of PWML group was obviously higher than that of DWML group(P<0.05).4.The peripheral ICAM-1 level of WMIL group was significantly higher than that of healthy controls(P < 0.05);The peripheral ICAM-1 levels of light,medium and heavy WMIL these three groups had differences(P < 0.05),correlation analysis showed that peripheral ICAM-1 level was positively correlated with Fazekas scores(P<0.05),and negatively correlated with Mo CA scale scores(P < 0.05).DWML and PWML group had no obvious difference in peripheral ICAM-level(P > 0.05).Conclusions:1.Peripheral anti-MOG antibody levels may has a certain role in the onsetand progression of WMIL,may be an important risk factor for WMIL,and it may have even more closely with PWML.2.Peripheral ICAM-1 level may play a role in the onset of WMIL,and may be a risk factor for WMIL.3.A certain degree of WMIL can decreased thecognitive function of patients,and the degree of cognitive impairment and WMIL severity may be positively correlated,and may be associated with the part of lesions.4.Age,smoking history,blood lipids and homocysteine may be the risk factors for WMIL.
