To Investigate The Value Of Transabdominal Ultrasoud In The Preoperative Assessment Of Adenocarcinoma Of Esophagogastric Junction

Objective To investigate the value of transabdominal ultrasoud(TUS)in the preoperative assessment of Adenocarcinoma of esophagogastric junction(AEG)and observe the morphological features of Adenocarcinoma of esophagogastric junction under the transabdominal ultrasoud.Methods Totally 65 patients from The First Affiliated Hospital of Anhui Medical University who received surgical resection with a preliminary diagnosis of AEG by gastroscopy biopsy from December 2014 to May 2016 were checked with TUS and CT preoperatively,and the results which included AEG classification,TNM stage,tumor differentiation and tumor size were then analyzed and compared with surgical pathologic finds.Results Referring to histological type and Siewert's classification,of all the postoperative pathological data,31 cases(47.7%)were Borrmann type ?;15 cases(23.1%)were Borrmann type ?;29 cases(44.6%)were poor differentiated adenocarcinoma;19 cases(29.2%)were Moderate differentiated adenocarcinoma;TUS and CT on the accurate diagnosis in AEG's clinical classification were 90.8%,92.3%;among which type?was 85.7%,92.9%;and II was 91.4%,88.6%;III was93.8%,100%.TUS and CT have consistency in diagnosis of AEG's clinical classification,and there was no significant difference between them(P<0.05).And the accuracy of TUS and CT in AEG's T-staging is respectively 33.8% and 63.1%.The diagnostic differences of the two have statistical meaning in the T-staging while there was no significant difference in N-staging and M-staging by CT and TUS(P>0.05),the accuracy for T-staging by CT was significantly higher than TUS(P<0.05),but for those who were preoperatively diagnosed as poor differentiated adenocarcinoma,there was no significant difference between TUS and CT(P>0.05).The longest diameter of the tumor measured by TUS was 4.00cm±1.84 cm,and the result of postoperative pathology was 4.41cm±2.52 cm,there was no significant difference between the two groups(P>0.05).Conclusion TUS is an available tool for the diagnosis of the type of AEG,which is useful for deciding surgical approaches;TUS has high value in preoperative N ?M-staging,and for patients who preoperatively diagnosed as poor differentiated adenocarcinoma,both TUS and CT can be used in preoperative TNM-staging.And TUS can be used as a reliable means to measure the size of AEG mass.It is of great significance to guide the operation and judge the prognosis.
