Objective:Investigating the diagnostic value of 320-Detector row CT perfusion imaging in common liver tumors.Methods:Sixty-seven patients with liver tumors examined by 320-Detector row CT perfusion imaging.The hepatic arterial flow(HAF),hepatic portal flow(HPF)and hepatic perfusion index in tumor area,peritumoral liver tissue and distal normal liver tissue were measured and the differences in perfusion values between different regions of the same patient and differences in perfusion values between hepatic carcinoma,hepatic metastases and hepatic hemangiomas were compared.Time density curve(TDC)of normal liver,hepatic carcinoma,liver metastasis and hepatic hemangioma were investigated.The effective dose(ED)of CT perfusion imaging of the liver and liver CT scan with three phase on patient were also investigated.Results:1.The HAF(105.94±31.22 mL/min/100mL)and HPI(77.62±15.64%)of tumor area in hepatic carcinoma group were significantly higher than that of peritumoral liver tissue(51.74±15.98 mL/min/100mL),(30.94±7.62%)and distal normal liver tissues(35.29±13.39mL/min/100mL),(16.26±5.19%),while the HPF(37.71±30.96 mL/min/100mL)of tumor area was significantly lower than that of peritumoral liver tissue(113.91±34.77mL/min/100mL)and distal normal liver tissues(184.74±40.57 mL/min/10OmL).There were statistical differences among different regions(P<0.05).2.The HAF(61.38±2.67 mL/min/100mL)and HPI(44.55±3.53%)of tumor area in liver metastases were higher than that of peritumoral liver tissue(43.92 ± 4.33 mL/min/100mL),(27.46±2.37%)and distal normal liver tissues(35.70±6.30 mL/min/100mL),(16.43±3.26%),while the HPF(77.33±13.88mL/min/100mL)of tumor area was lower than that of peritumoral liver tissue(116.04±8.74mL/min/100mL)and distal normal liver tissues(185.28±30.40mL/min/100mL).There was statistical difference between the three regions(P<0.05).3.The HAF(203.56±44.61 mL/min/100mL)and HPI(82.01±7.69%)of tumor area in hepatic hemangioma were significantly higher than that of peritumoral liver tissue(34.87±9.08 mL/min/100mL),(16.08±3.85%)and distal normal liver tissues(35.82±9.43 mL/min/100mL),(16.18±3.96%).The HPF(39.05±18.03 mL/min/lOOmL)of the tumor area was significantly lower than that of the peritumoral liver tissue(189.43±36.06 mL/min/100mL)and distal normal liver tissue(188.68±38.46 mL/min/100mL).There were statistical differences between the three regions(P<0.05).4.The HAF,HPF and HPI value of tumor area in hepatic carcinoma,liver metastases and hepatic hemangioma had statistical difference(P<0.05).The perfusion parameters of peritumoral liver tissue had statistical difference between the three types of tumors(P<0.05).There were no statistical differences in perfusion parameters of normal liver tissue in three kinds of tumor(P>0.05).5.The TDC curve in normal liver tissue increased to the peak curve at 32± 1.4 s,which showed a slow rise and slow drop type.The TDC curve in liver cancer increased to the peak curve at 20.17 ± 1.6 s,which showed a speed rise and speed fall type.The TDC curve in liver metastasis increased to the peak curve at 20.0 ± 1.0 s,the type was speed up and slow down and its peak value was lower than that of liver cancer.The TDC curve in hepatic hemangioma increased to the peak curve at 18.6 ± 1.7 s,the type was speed up and slow down and its peak value was significantly higher than that of liver cancer.6.The ED of CT perfusion imaging of the liver and liver CT scan with three phase on patient was 13.04 ± 2.35 mSv and 36.39 ±7.32 mSv respectively.There was significant statistical difference between the two groups(t=-10.81,P=0.000<0.05).Conclusion:1.The 320-Detector row CT perfusion imaging could be used to quantify the hemodynamics of common liver tumors and had important value in the diagnosis of tumors.2.The perfusion parameters value of peritumoral liver tissue was of great value in the differential diagnosis of benign and malignant tumors?judgment of true range of tumor and evaluation of tumor tissue infiltration.3.The TDC curve was of great significance in the diagnosis and differential diagnosis of liver tumors.4.The ED of the 320-Detector row CT perfusion imaging on patients was significantly lower than that of liver CT scan with three phase.
