Exploring The Rational Applicat Ion Of Proton Pump Inhibitor In Clinical

Objective1.Summaring and analyzing the unreasonable use of proton pump inhibitors(PPIs)and adverse reactions;2.Formulating the evaluation criteria of proton pump inhibitors;3.Investigating the use of proton pump inhibitors in our hospital,and analyzing and evaluating the rationality application of PPIs,based on the evaluation criteria of PPIs;4.Formulating the principle of proton pump inhibitors in our hospital to promote the rational application of proton pump inhibitors Methods1.Summaring and analyzing the phenomenon of irrational use of PPIs and the adverse reactions by reviewing the CNKI datebase,wanfang datebase,Vip datebase,pubmed,Web of science and so on,based on keywords such as proton pump inhibitors,adverse reaction of Proton Pump Inhibitors,reasonable application of proton pump inhibitors;2.Formulating the evaluation criteria of proton pump inhibitor through access to relevant literature,drug description;3.Using retrospective investigation research methods,randomly selected patients who had used PPIs in our hospital from January 2015 to March 2016 to analyze and evaluate the rational application of PPIs;4.The principle of using PPIs in our hospital was formulated according to the use of PPIs in our hospital.Results1.Proton pump inhibitors have the phenomenon of unreasonable application,such as overdose use,the course of treatment and the frequency unreasonable use,no indications use and so on,both common and long-term use of potential adverse reactions occur more.2.In this study,the randomly selected 2805 cases,intravenous administration 1241 cases(44.20%),Oral administration 1564 cases(55.80%);Among the 2805 patients,only 232(8.27%)cases for treatment use,they all have clear indications,such as duodenal ulcer,upper gastrointestinal bleeding,gastric ulcer;2179(77.68%)cases for prophylaxis use,of the 2179 prophylaxis cases,1092 cases(58.86%)have recommended indications,1087(49.89%)have no indications;394(14.05%)cases for no clear use purpose and have no indications.The Overdose use,the course of treatment and the frequency unreasonable use rate of PPIs are all low,the healing period unreasonable use are injection preparation.248 cases(8.84%)have indications,and non-indications 2557(91.16%)among the 2805 patients.There are 8 factors influencing the application tendency of PPIs.ConclusionThe intravenous administration rate is high,nearly half have no indications;The proportion of prophylaxis use is higher than treatment use,and the percentage of no indications in prophylaxis use is high;No indications use is becoming more and more serious,the ratio of non-indications to indications was about 10.0:1.0.Irrational use of PPIs will increase the incidence of adverse reactions and medical expenses.Clinicians and clinical pharmacists should pay attention to proton pump inhibitors common and long-term use of potential adverse reactions,rationaly use PPIs based on the formulated principles of PPIs to reduce the adverse reactions and unreasonable application of PPIs.
