| ObjectiveTo explore the significance of CC16 and the severity in patients with community acquired pneumonia and the possibility of CC16 serving as a diagnostic biomarker of severity for CAP.MethodsNinety-two patients with CAP between Oct.2014 and Oct.2015 in our hospital were enrolled.All patients were divided into low-risk,moderate-risk or high-risk groups according to PSI and CURB-65 score systems,and also divided into non-severe pneumonia group,severe pneumonia group and death group according to the "Guidelines for the diagnosis and treatment of community acquired pneumonia".For PSI score system,there were 33 patient for low-risk group,42 patient for moderate-risk group and 17 for high-risk group,which were 70,13 and 9 for CURB-65 score system,accordingly.For the guideline system,there were 58 patients for non-severe group,28 patients for severe group and 6 patients for death group.Forty healthy controls(20 male and 20 female)were also enrolled.Blood samples were collected in CAP patients and control subjects.The level of CC16 was measured by enzyme-linked immunosorbent assay(ELISA).Results1.92 patients were divided into non-severe group,severe group and death group.The scores of PSI and CURB-65 were highly correlated with the severity of patients.The PSI scores for all 3 groups are 88.64± 11.09,133.45± 14.29 and 161.241±18.06.the CURB-65scores for all 3 groups are 0.93 ± 0.16,1.83 ± 0.74 and 2.81 ± 1.05.The scores of PSI and CURB-65 for death group are significantly higher than that for non-severe group and severe group,which for severe group is also higher than that for non-severe group(P<0.05).2.All patients were divided into low-risk group,moderate-risk group and high-risk group according to PSI score system.The expression of CC16 were down-regulated with more severe pneumonia.The expression levels of all 3 groups are(10.21 ±3.01)μg/L,(8.23±2.05)μg/Land(6.22±1.48)μg/L in the serum of patients.The expression of CC16 in high-risk group is significantly lower than that in moderate and low risk groups,and the CC16 expression in moderate-risk group is lower than that in low-risk group(P<0.05).3.All patients were divided into low-risk group,moderate-risk group and high-risk group according to CURB-65 score system.The correlation between the CC16 expression and the severity by CURB-65 score system is similar to that by PSI scoresystem.The expression levels of all 3 groups are(9.24±2.21)μg/L,(7.26±1.42)μg/L and(5.24±1.48)μg/L in the serum of patients.The expression of CC16 in high-risk group is significantly lower than that in moderate and low risk groups,and the CC16 expression in moderate-risk group is lower than that in low-risk group(P<0.05).4.There was significant negative correlation between the concentration of CC16 expression and PSI score(P<0.05,r=-0.418)and CURB-65 score(P<0.05,r=-0.217).And the correlation between CC16 and PSI score system is better than that between CC16 and CURB-65 score system.5.The CC16 expression in patients before treatment is significantly lower than that in the healthy control people(P<0.05).After treatment,the CC16 is dramatically increased when compared to that before treatment(P<0.05).Conclusion1.The expression of CC16 decreases as the stronger severity of CAP patient,which is also negatively correlated with the PSI/CURB-65 scores.2.The concentration of CC16 in the serum is negatively correlated with the PSI/CURB-65 scores and severity of CAP,which indicates that CC16 might be used as a diagnostic biomarker in the CAP. |
