Objective:To investigate the clinical factors,treatment and prognosis of patients by reviewing the clinical data of patients with inflammatory bowel disease and to explore the related risk factors influencing the prognosis in order to improve their clinical work and get better guidance of clinical diagnosis and treatment.Methods:Based on the literature review of pathogenesis and diagnosis and treatment of inflammatory bowel disease in western medicine and traditional Chinese medicine,the clinical retrospective study was used to collect and arrange the clinical data of 450 patients with inflammatory bowel disease admitted between August 2009 and November 2016 from the adjuvant hospital of the Third Affiliated Hospital of Nanjing University of Traditional Chinese Medicine,included general condition,medical history,TCM syndromes and treatment.Then follow-up to 142 cases of ulcerative colitis patients and 81 cases of Crohn's disease patients by telephone to know its prognosis.Statistical analysis use SPSS 19.0 software.Result:1.Clinical data(1)Age and gender:There were 176 males and 191 females with ulcerative colitis,male and female ratio was about 1:1.09.There were 55 males and 28 females with Crohn's disease,male and female ratio was about 1.96:1.The average age of patients with ulcerative colitis was 43.33± 13.41 years old,patients aged 30-39 are the most,the average age of Crohn's disease patients was 34 ± 12.62 years old,patients aged 20-29 are the most;(2)Living in different places:There were 17%of the total number of inflammatory bowel disease patients with long-term residential history,62 cases of ulcerative colitis patients and 15 cases of Crohn's disease patients.(3)Lesions:About 56%ulcerative colitis patients are involved the left colon(including straight sigmoid colon),55%Crohn's disease are involved the small intestine(terminal ileum);(4)Clinical type:The initial(32.9%)and chronic recurrence type(62.7%)is more in ulcerative colitis patients;(5)TCM syndromes:ulcerative colitis with spleen deficiency and dampness syndrome(32.9%),liver and spleen deficiency syndrome(24.8%),large intestinal damp-heat syndrome(24.3%)more common,Crohn's disease with damp-heat intrinsic syndrome(32.5%),liver and spleen deficiency syndrome(25.3%),spleen and kidney yang deficiency(20.5%)are more common;(6)Clinical manifestations:pus and blood stool(76%),diarrhea(75.5%),abdominal pain(69.2%)are the main symptoms of ulcerative colitis,abdominal pain(45.8%),diarrhea(43.3%),mucous stool(40%)are the main symptoms of Crohn's disease;(7)Inflammatory bowel disease parenteral manifestations:The incidence is not high and the incidence of Crohn's disease in perianal lesions(74.7%)was higher than that in ulcerative colitis(22.1%);(8)Treatment:Chinese medicine:after dialectical,the main use of traditional Chinese medicine was decoction oral and ulcerated enema,Western medicine mainly use amino salicylic acid preparations with micro-ecological agents,.According to the situation,use glucocorticoids,antibiotics,etc.2.Prognostic analysisThe use of COX proportional risk model,found that the diet,abdominal surgery history,clinical type,severity of illness,different TCM syndromes and whether the decoction of traditional Chinese medicine can be used as an independent prognostic factor of ulcerative colitis;age,diet,abdominal surgery history,the extent of the disease,severity of illness and whether the decoction of traditional Chinese medicine can be used as Crohn's disease independent prognostic factors.Conclusion:1.The general data and clinical characteristics of inflammatory bowel disease patients in this study are consistent with those reported at domestic and overseas,but there are also differences,which may be due to different sample capacity,regional differences,ethnic differences,etc.Treatment was dominated by internal medicine,and characterised of Chinese medicine oral and kuijie liquid enema,perianal Crohn's disease combined with surgical treatment.2.A variety of factors affect the prognosis of IBD,clinical diagnosis and treatment can accord these factors to develop individualized treatment program.
