Purpose:To compare the diagnostic value of conventional ultrasonography and contrast-enhanced ultrasound in the diagnosis of hepatic solid space occupying lesions and to explore the value and influencing factors of guided percutaneous transhepatic biopsy.Method:This study selected June 2012-2016 June treatment of liver patients lesions in our hospital,a total of 89 cases,95 lesions were randomly divided into conventional group and angiography group,of which the conventional group of 53 lesions,42 cases of contrast group,the conventional ultrasound and contrast-enhanced ultrasonography for hepatic solid lesions of the diagnosis and pathological results,puncture results and pathological results were compared,and analysis of the basic situation of the lesion on the impact of puncture results.Result:The accuracy of the conventional diagnostic group was 71.6%,and the accuracy rate of the imaging group was 90.5%.The accuracy rate of the two groups was (?)~2= 5.164,P <0.05,Differences were statistically significant.Group routine diagnostic sensitivity was 75.6%,the diagnostic imaging group sensitivity was 91.2%,two groups of data sensitivity comparison,(?)~2 = 3.143,P(29)0.05,no statistically significant difference;Regular group specificity is 58.3%,contrast group specificity of 87.5%,specificity compared two groups of data,(?)~2 = 5.787,P<0.05,the difference is statistically significant.Conventional group puncture accuracy is 77.4%,contrast group puncture accuracy rate was 95.2%,the accurate data for the two groups,(?)~2 = 5.691,P < 0.05,the difference is statistically significant;Group puncture sensitivity was 80.5%,imaging the sensitivity was 97.1%,two groups of data sensitivity comparison,(?)~2 = 4.833,P < 0.05,the difference is statistically significant;Puncture group puncture specificity of 66.7%,imaging group specificity of 87.5%,specificity compared two groups of data,(?)~2 = 1.111,P(29)0.05,no statistically significant difference.The accuracy of diameter<2 cm and diameter?2 cm in normal group were 31.25% and 97.2%,respectively.The accuracy of the two groups was different,(?)~2= 27.819,P <0.05,and the difference was statistically significant.When the diameter of the lesion was less than 2 cm,The accuracy rate of puncture was 90.9% in the contrast group and 31.25% in the conventional group.The accuracy of the two groups was different,(?)~2 = 9.396,P <0.05,the difference was statistically significant.The accuracy of puncture in the clear group was 93.1% and 58.3%,respectively.The accuracy of the two groups was different,(?)~2 = 9.064,P <0.05,the difference was statistically significant;when the lesion boundary was not clear,The accuracy rate of puncture was 94.7% in the contrast group,58.3% in the conventional group,and the accuracy of the two groups was (?)~2 = 7.382,P <0.05,the difference was statistically significant.The accuracy of puncture was 95.2% and 65.6% respectively.The accuracy of the two groups was different,(?)~2 = 6.348,P <0.05,the difference was statistically significant.When the internal echo of the lesion was In the case of nonuniformity,the accuracy rate of puncture was 95.0% in the contrast group and 65.6% in the conventional group.The accuracy of the two groups was different,(?)~2 = 5.983,P <0.05,the difference was statistically significant.The accuracy rate of puncture in the conventional group was 60.7% and 96.0% respectively compared with that from the liver surface ?5cm and the hepatic surface <5cm,respectively.The accuracy of the two groups was different,(?)~2 = 9.389,P <0.05,the difference was statistically significant;The accuracy rate of puncture was 94.4% and 60.7% in the conventional group.The accuracy of the two groups was different,(?)~2 =6.465,P <0.05,the difference was statistically significant.The accuracy of puncture in the liver of the patients with liver cirrhosis and liver cirrhosis was 55.0% and 90.9% respectively.The accuracy of the two groups was different,(?)~2 =9.168,P <0.05,the difference was statistically(P <0.05).The accuracy rate of puncture in the treatment group was 94.4%,and the accuracy rate of puncture in the conventional group was 55.0%.The accuracy of the two groups was different,(?)~2 =7.602,P <0.05,the difference was statistically significant.In conclusion:The accuracy of ultrasonography in the diagnosis of solid liver lesions was higher than that of conventional ultrasonography.Ultrasonography showed that the accuracy of percutaneous transluminal biopsy was higher than that of conventional ultrasonography.Routine ultrasound guided liver biopsy,The diameter of ? 2cm than the diameter of <2cm puncture biopsy accuracy is high;border clear than the border is not clear puncture biopsy accuracy is high;internal echo even less than the internal echo uneven biopsy accuracy;puncture depth <5cm More puncture depth ? 5cm puncture biopsy accuracy is high;background liver parenchymal cirrhosis without cirrhosis with high accuracy of biopsy.When the liver solid lesion diameter <2cm,the border is not clear,the internal echo is not uniform,puncture depth ? 5cm,liver background without liver cirrhosis,ultrasound angiography to guide the accuracy of biopsy than conventional ultrasound.
