Objective:Over the past few decades,the incidence of colorectal cancer in our country gradually increased,about 15%-25% of patients with colorectal cancer in the diagnosis of a merger with liver metastases,and another15%-25% of patients with colorectal cancer The primary tumor for radical surgery after the development of liver metastases,surgery R0 resection of primary lesions and metastases is possible to cure colorectal cancer liver metastasis(CRCLM)the best means.At present,the initial resectable colorectal cancer patients with simultaneous liver metastases to take concurrent surgical resection or staging surgery is still controversial.This study analyzes the safety and near-long-term effects of surgical resection and staging in the same period,so as to provide the best surgical timing and operation mode for the clinician to provide the thoughts and theoretical basis for the simultaneous treatment of colorectal cancer.Methods:A total of 71 patients with simultaneous resection of colorectal cancer(SCLM)with metastatic colorectal cancer were enrolled in this study from September 2006 to September 2011,among which 33 patients underwent simultaneous surgical resection(simultaneous surgery group),38 patients with primary colorectal resection and then resection of liver metastases(primary colorectal surgery group).Analysis of the two groups of patients in the amount of bleeding,operation time,the incidence of postoperative complications,total length of stay and survival rate differences.Results:Both groups were discharged smoothly.The average amount of bleeding in the simultaneous surgery group was 476.64 ± 118.33 ml,the primary colorectal surgery group was 559.55 ± 111.78 ml.The simultaneous surgery group operation time was 229.81 ± 19.33 min,the primary colorectal surgery group was 261.92 ± 21.97 min.The total hospital stay was 16.91 ± 2.99 days in the simultaneous surgery group and 19.91 ± 2.64 days in the primary colorectal surgery group,and the difference was statistically significant(р<0.05).The incidence of postoperative complications was 24.2% in simultaneous surgery group and 18.4% in the primary colorectal surgery group.Although the complication rate in the simultaneous group was higher than that in the primary colorectal surgery group,the difference was not statistically significant(р> 0.05).The The median survival time was 35 m in the simultaneous surgery group and 38 m in the primary colorectal surgery group.The survival rates were 90.9%,48.5% and 30.3% in the simultaneous surgery group,and 89.5%,52.6%,28.9% in the primarycolorectal surgery group,the difference between the two groups is not statistically significant,long-term effect is quite.Conclusion:Simultaneous surgical treatment of resectable colorectal cancer patients with simultaneous liver metastases is safe and effective,compared with primary colorectal resection surgery can reduce intraoperative blood loss,operation time,total length of stay,with socio-economic benefits,it is worthy of promotion.
