Objective:(1)Comparison of clinical data of Han and Uygur patients with hydatidiform mole in Xinjiang areas(2)to explore the diagnostic value of E-cadherin and Laminin in different types of hydatidiform mole(hydatidiform mole and invasive mole);(3)To retrospectively analyze the high risk factors of clinical data of malignant patients with hydatidiform mole.Materials and methods:(1)A total of 167 patients with hydatidiform mole were selected from March 2010 to March 2015 at the FirstAffiliated Hospital of Xinjiang Medical University,and their clinical data such as age and ethnic composition were analyzed.(2)A total of 17 patients with gestational trophoblastic tumor(GTN)(including 13 cases of invasive hydatidiform mole and 4 cases of choriocarcinoma)were enrolled in the study.gestatioal time,gestational age,serum ?-HCG level,uterine size,malignant transformation and other factors were retrospectively studied;(3)Immunohistochemical SP method was used to detect the expression of E-cadherin and Laminin in paraffin-embedded tissues of 75cases of hydatidiform mole in our hospital.Results:(1)E-cadherin andLaminin are expressed in partial hydatidiform mole,complete hydatidiform mole,and invasive hydatidiform mole.(2)The expression of E-cadherin and Laminin in hydatidiform mole and invasive hydatidiform mole were different,and the difference was statistically significant(P <0.05)(3)In 167 cases of hydatidiform mole cases,including 107 cases of Han nationality,the proportion was 64.1%;Uygur 60 cases,the proportionwas 35.9%,combined with the total number of pregnancy occurred in our hospital of study period.There was a significant difference between the two groups(X~2= 14.844,P <0.001)(4)167 patients,33 cases had no spontaneous abortion history,accounting for 19.8%;42,cases had one history of abortion,accounting for 25.1%;92 cases had two or more miscarriages history,accounting for 55.1%,After statistics processing,there was a significant difference between ?two miscarriages history and< two history of miscarriage,statistical significance was found(mean P <0.001),but there was no significant difference between the one history of miscarriage and none history of miscarriage(P = 0.180);(5)167 cases of hydatidiform mole were followed up for 18 months,including uterine uterus size> 10 weeks were 42 cases,8 cases of malignant transformation,malignant transformation rate was 19.0%;uterine uterus size ?10 weeks were 125 cases,9 malignant transformation,malignant transformation rate was 7.2%.The gestational age of the two groups was statistically significant(X~2 = 4.826,P <0.05);(6)Among the 17 malignant patients,15 were Han,the malignant rate was 14.0%,the Uygur was 2,and the malignant rate was 3.3%.There was a statistically significant difference between the two groups(X~2= 4.801,P <0.05);(7)Among the 167 hydatidiform mole cases,32 cases of HCG <10~5MIu / ml had 1 case ofmalignant transformation,the rate of malignancy was 3.1%,62 cases of HCG level 10~5MIu/ml to 2×10~5MIu/ ml had 2 cases of malignant transformation,the rate of malignancy was 3.2%,73 cases of HCG level>2×10~5MIu /ml had 14 cases of malignant transformation,the rate of malignancy was 19.2%,There was no statistically significant difference between the HCG level of <10~5MIu / m L and HCG level of 10~5-2×10~5MIu / m L(X~2 = 0.001,P = 0.979),the HCG level of 10~5-2×10~5MIu / ml were statistically significantly different from those of> 2×10~5MIu / ml(X~2= 8.166,P = 0.004),the HCG level of <10~5MIu / ml were statistically significantly different from those of> 2×10~5MIu / ml(X~2 = 4.682,P= 0.030).Conclusions:(1)E-cadherin and Laminin are associated with the malignant potential of trophoblast cells;(2)Immunohistochemical analysis of E-cadherin and Laminin can be used as an adjunct to the traditional diagnosis of different types of hydatidiform mole.(3)The expressionof E-cadherin and Laminin was negatively correlated with the degree of malignancy of trophoblastic cells.(4)The risk of suffering from hydatidiform mole was higher in Uygur people than the Han people;(5)The proportion of hydatidiform mole is higher in people who had two or more miscarriages history;(6)hydatidiform mole was more likely to deteriorate in those cases whose uterine size> 10 weeks before curettage(7)the risk of hydatidiform mole malignancy in Han patients was higher than Uygur(8)those hydatidiform mole patients whose serum HCG level of more than 2 × 10~5 MIu / ml before curettage have a higher chance of malignant transformation.
