Objective To explore the feasibility of Bone-soft tissue Combined transport surgery in the treatment of tibial bone and soft tissue defect and evaluated the clinical value of the technology.Methods Retrospectiveily analysised 10 patients from Fuyang Clinical Medical College of Anhui Medical University Clinical with tibial bone and soft tissue defect treated with Bone-soft tissue combined transport surgery between October 2011 to July 2016.10 patients were treated by Bone-soft tissue Combined transport surgery.7 males,3 females,aged from 25 to 59 years,with an average age of 38.2 years.There were 6 cases in left side and 4 cases in right side,The reasons of injury included high energy injury(2/10)and chronic inflammation of the tibia(8/10).The length of bone defect ranged from 5 to15 cm with an average of 9.1cm and the area of soft tissue defect ranged from 3 to 8cm*9 to 23 cm with an average of 5cm*13cm.The mean follow-up period was 22 months(6-35months).Clear focus of infection,Resect of bone disease,Install Ilizarove external fixation.Bone-soft tissue Combined transport surgery was lged in treatment of tibial bone and soft tissue defect.With the transport of bone,two bone defects were aligned and soft tissue gradually closed.When X-ray examination showed that the two bone defects were aligned,The Ilizarov external fixator was gradually simplified.According to the bone healing and the mineralization.After the tibia was healed,the external fixation was removed.The Paley score was used to evaluate the clinical outcome of the patients.The excellent and good rate and complications,ankle,knee function were evaluated.Results All the patients were followed up for 6 months to 32 months,with a mean followed-up an average of 22 months.The infection were controlled and tissue defects were repaired,no re-fracture occurred.All the knee and ankle joints were good.There were 2 cases suffered postoperative pin tract,one of them Infection was controlled with the application of antibiot infection.The other one was controlled with Changed fixed orbit and debrided,1 femoral neck nonunion.According to the Paley score,8cases were excellent,2were good.Conclusion Bone-soft tissue Combined transport surgery is an optional method to treat tibial bone and soft tissue defect,the infection were controlled,With the bone and soft tissue transport the wound was closed,Simultaneously,So as to achieve the simultaneous treatment of the tibia defect with skin defect,It has the advantages of easy operation,minimum trauma,wide indication and so on.
