Objective: To non-invasively monitor regional cerebral oxygen saturation?rScO2?in patients with severe brain injury after operation which were continuously monitored by near infrared spectroscopy?NIRS?and investigate the correlation between the changes of regional cerebral oxygen saturation and intracranial pressure?ICP?.Methods: To select 24 patients with Severe brain injury after emergency operation in anqing affiliated hospital of anhui medical university from 2015.july to 2016.november.And intracranial pressure by near infrared spectroscopy and invasive intracranial pressure monitor.Meanwhile used multi-parameter ecg monitor to monitoring pulse oxygen saturation?SpO2?,mean arterial blood pressure?MAP?and cerebral perfusion pressure?CPP?.The correlation of monitoring results was analyzed by SPSS statistics analysis.Results: The prognosis of severe brain injury and rScO2 statistically significant.The rScO2 reduced in the early postoperative period.As the intracranial pressure relieves it increases.But the rScO2 in 4 cases of death decline as others subsequently it increased while the ascending trend was not obvious.A significant negative correlation between the rScO2 and ICP?r=-0.882,P<0.001?,and a positive correlation between the rScO2 and CPP?r=0.938,P<0.001?.The MAP associated with rScO2,ICP,CPP has no statistical significance of linear?P>0.05?and the values of P were 0.173,0.135,0.171,respectively.In patients with severe head injury treatment process,the MAP,SpO2 maintained within the normal range,Only gradually reduce while the patient is in a dying condition.Conclusions: Presently invasive monitoring is widely used in clinic,but it is expensive,and with the risk of intracranial infection,hemorrhage and even death.And NIRS technology has the characteristic such as noninvasive,convenient and precise,it can accurately and non-invasively monitor rScO2 which is particularly sensitive to oxygen.RScO2 can timely reflect whether there is Ischemia oxygen metabolism After severe brain injury.So rScO2 can serve as a index to estimate the change of the patients' condition.By the use of NIRS there is great significance to research severe head injury treatment and make prognosis assessment.
