The Study Of The Recovery Of Gastrointestinal Function By Electroacupuncture Combined With Evodia Hot Compress In Postoperative Cholelithiasis Treated With Minimally Invasive Surgery Based On The Concept Of Enhanced Recovery After Surgery

ObjectiveThe topic is based on the concept of Enhanced Recovery After Surgery.The patients of extrahepatic biliary calculi underwent minimally invasive treatment after operation,were treated with electroacupuncture at Zusanli and Shangjuxu,and with Evodia hot compress the abdomen,then observe the safety indicator and the postoperative recovery of gastrointestinal function,which includes the anal exhaust and defecation time,and recovery time of full flow,then the systematic evaluation and to explore its clinical significance in fast track surgery in the treatment of electroacupuncture combined with evodia hot compress,provide the basis for the treatment of clinical application of TCM External Therapy of postoperative gastrointestinal function accelerate postoperative rehabilitation.MethodsSelect the patients admitted to hospital treatment of the Department of hepatobiliary surgery of the First Affiliated Hospital of Guangzhou University of Chinese Medicine(surgical III clinic)from December 2016 to January 2016.60 Cases of patients with extrahepatic cholelithiasis in the treatment of minimally invasive surgery were collected.Patients were divided into treatment group(acupuncture Zusanli,Shangjuxu combined Evodia hot compress group)30 cases,control group(conventional treatment group)30 cases using simple random method.During the perioperative period,all the patients were treated with the concept of Enhanced Recovery After Surgery,including the preoperative and postoperative measures.Before operation,all patients accepted the preoperative education,shorten the fasting time,without bowel preparation.After operation,the control group was given routine treatment;including:the routine surgery postoperative nursing of the Department of hepatobiliary,the use of antibiotics,the protection of liver and stomach,conventional nutritional support,maintaining water and electrolyte balance.On the basis of control group treatment,treatment group was treated with electroacupuncture at Zusanli and Shangjuxu combined with the Evodia hot compress the abdomen treatment.Then observed and recorded the safety indicators and relevant indicators of the recovery of gastrointestinal function after operation in the two groups,combined with the clinical observation table to collect data for statistical analysis and summary of the data of two groups,to verify the curative effect of the treatment with electroacupuncture combine with Evodia hot compress the abdomen.Result1.The basic information and surgical data of the two groups were compared:there was no significant difference between the experimental group and the control group;2.The comparison of the recovery of gastrointestinal function data of the two groups after surgery:compared with the control group,the patients of experimental group spend less time to recovery exhaust defecation of anus,and recovery full liquid diet,the difference was statistically significant(P<0.05).The postoperative hospitalization days and hospitalization time were shorter than the control group,but the difference was not statistically significant(P>0.05).3.The comparison of the safety indicators:there was no perioperative death or anesthesia and cardiovascular events in the two groups.In the 1 days before operation,the frist and third days after operation,test the blood routine,liver function,renal function,electrolyte detection,and compare the number of white blood cells(WBC),neutrophil percentage(NEUT%)and other major blood indexes;alanine aminotransferase and aspartate aminotransferase(ALT)(AST),total bilirubin(TB)and liver function indices,serum creatinine(CR)and renal function indices;potassium ion(K+)electrolytes such as main index,the result shows that P>0.05,there is no obvious difference between the two groups.4.By the t test,chi square test and variance analysis,the experimental group and the control group,the experimental group was better than the control group in promoting the recovery of gastrointestinal function after abdominal surgery.ConclusionElectroacupuncture at Zusanli,Shangjuxu combined with abdominal Evodia Compress treatment of extrahepatic cholelithiasis in minimally invasive surgery is safe and reliable,improve postoperative gastrointestinal function has a positive effect,and can improve the postoperative gastrointestinal symptoms to a certain extent.
