The Value Of Platelet Glycoprotein ?b/?a In The Diagnosis And Prognosis Of Immune Thrombocytopenia

Objective:To investigate the positive expression rates of platelet glycoprotein ?b/?a(GPIIb/?a)in patients with primary immune thrombocytopenia(ITP),non-immunotic thrombocytopenia and normal controls Plasma GP?b/IIIa concentration level,and analysis of both the expression and clinical significance for the diagnosis of primary immunosuppressive thrombocytopenia and prognosis to provide clinical value.Methods:Respectively using flow cytometry(Flowcytomtry,FCM)and Enzyme Linked ImmunoSorbent method(Enzyme-Linked ImmunoSorbent Assay,ELISA)detection of 20 patients with primary immune thrombocytopenia and non immune thrombocytopenia in 20 cases and 20 cases of normal control group patients with positive expression rate of platelet glycoprotein IIb/IIIa and GP?b/IIIa concentration in plasma levels.Conclusion:Platelet counts and positive expression rate of GPIIb/IIIa(the CD41+/CD61+)ofpatients with ITP and non-immune thrombocytopenia were significantly lower than normal controls(p<0.05)(The results platelet count in ITP is 16.15±17.56×109/L,non-ITP is 19.35±8.62×109/L,normal control is 221.95±18.98×109/L;positive expression rate of GP?b/?a in ITP is 33.39±15.70%,non-TIP is 51.53±20.12%,normal control is 94.34±4.47%);There were no significance(p >0.05)of platelet counts between ITP and non-ITP,but the difference of positive expression rate of GPIIb/IIIa beween them are statistically significant(p<0.05);The concentr-ationl evels in plasma of GP?b/IIIa of patients with ITP and non-immune throm-bocytopenia were significantly lower than the normal controls(p<0.05)(The results GP?b/IIIa concentration in ITP is 22.61±13.63 ng/ml,non-ITP is 19.06±8.36 ng/ml,normal control is 53.53±3.86 ng/ml),There were no significance(p >0.05)of concentrationl evels in plasma of GP?b/IIIa between ITP and non-ITP.The expression of GPIIb/IIIa in in ITP group was positively correlated with platelet count(r = 0.869,p <0.05).The platelet GPIIb/IIIa concentration in ITP group was positively correlated with the number of platelet count(r = 0.924,p <0.05).The The ROC curve of platelet GPIIb / IIIa positive expression rate in ITP patients was obtained by using SPSS software and GPIIb / IIIa positive expression rate of non-ITP group.The sensitivity and specificity of the curve were 43.55 % Was about the index,and the sensitivity was 80%,the specificity was 70%,the positive likelihood ratio was 2.67,and the positive predictive value was 91%.
