Part I The Comparative Study on COPD Patients and Normal Persons with Dual Source CT Dual Energy Lung Perfusion Imaging Automatic Quantitative Evaluationobjective To investigate whether there are differences between COPD patients and normal persons with dual source CT dual energy lung perfusion imaging automatic quantitative evaluation.Materials and Methods 18 cases of no abnormal persons were control group;62 cases of patients were COPD group:Combining the 2015 edition of Global Initiative for Chronic Obstructive Lung Disease(GOLD),COPD group was divided into four groups according to severity(GOLD I group was 15 cases,GOLD II group was 18 cases,GOLD III group was 17 cases,GOLD ? group was 12 cases).Siemens Syngo.via workstation dual energy software was used to make automatic quantitative analysis of relative enhancement of the whole and the local pulmonary perfusion.Apply SPSS20.0 software to make statistical analysis.Measurement data was expressed in mean differences±standard deviation(x±s).Variance analysis was used among the groups,and the pairwise comparison of the average number was performed with LSD method(the least significant difference method).P?0.05 was statistically significant difference.Results There was significant difference between the relative enhancement of local pulmonary perfusion in the control group and the COPD group(P<0.05).There was significant difference between the relative enhancement of the whole pulmonary perfusion in the control group and the COPD group(P<0.05).There was significant difference between the whole lung relative enhancement value in the GOLD I group and the GOLD III group?the GOLD IV group;There was significant difference between the whole lung relative enhancement value in the GOLD II group and the GOLD III group?the GOLD IV group;There was significant difference between the whole lung relative enhancement value in the GOLD III group and the GOLD IV group(P<0.05).There was no significant difference between the whole lung relative enhancement value in the GOLD I group and the GOLD II group(P>0.05).Conclusion The relative enhancement value of whole lung and local pulmonary perfusion with dual source CT dual energy lung perfusion imaging has an obvious difference between the COPD patients and normal persons.Dual source CT dual energy lung perfusion quantitative evaluation can be used to distinguish between COPD patients and normal persons.The severity of COPD lung parenchyma destruction can be evaluated.Part ? The Study on the Automatic Quantitative Evaluation of COPD Patients with Dual Source CT Dual Energy Lung Perfusion Imaging and the Correlation between Pulmonary Function and Pulmonary Emphysema Index AbstractObjective To investigate the relations between the lung perfusion relative enhancement value with dual source CT dual energy lung perfusion imaging automatic quantitative evaluation and the pulmonary function parameters and the emphysema index.To assess the whole and regional lung functions of COPD patients.Materials and Methods 62 COPD patients were carried on dual source CT dual energy lung perfusion enhancement scanning and pulmonary function examination.Siemens Syngo.via workstation dual energy software was used to make an automatic quantitative evaluation to the relative enhancement value of the whole and regional pulmonary perfusion and emphysema index.Pulmonary function parameters were obtained using pulmonary function detector.The main indexes measured were forced vital capacity(FVC),the first second forced expiratory volume(FEV1),the first second forced expiratory volume accounted for the ratio of forced vital capacity(FEV1/FVC)and The percentage of measured and predicted values of forced expiratory volume(FEV1/Pred).Apply SPSS20.0 software to make statistical analysis.Pearson correlation was used to analyze the correlation between the pulmonary function parameters and the relative enhancement of the whole pulmonary perfusion and the emphysema index,and the correlation between the regional lung relative enhancement value and the regional emphysema index.The definition of P<0.05 was statistically significant.Measurement data was expressed in mean differences ± standard deviation(x±s).Results The whole lung relative enhancement value were positively correlated with FEV1(r=0.480,P=0.025),FEV1%predicted value(r=0.601,P=0.001)?FVC(r?0.526,P=0.011)?FEV1/FVC(r=0.539,P=0.010).There was a negative correlation between the whole lung emphysema index and FEV1(r=-0.574,P=0.004)?FEV1/Pred(r=-0.605,P=0.002)?FVC(r=-0.572,P=0.004)?FEV1/FVC(r=-0.474,P=0.022).There was a negative correlation between the whole lung relative enhancement value and the whole lung emphysema index(r=-0.605,P=0.000).The relative enhancement of every regional lung was moderate negatively correlated with the corresponding regional emphysema index(P<0.05).Conclusion The automatic quantitative relative lung enhancement value of COPD patients using dual source CT dual energy lung perfusion was correlated with pulmonary emphysema index and pulmonary function parameters,which can be used to assess the degree of lung parenchyma destruction and airway obstruction.The relative enhancement of regional lung was moderately correlated with the regional emphysema index,which can be used to match the parenchymal destruction area and regional hypoperfusion.COPD patients with dual source CT dual energy lung perfusion automatic quantitative software can quickly carry out independent assessment to the whole lung or regional lung perfusion and emphysema.
