Effects Evaluation Of Simple Rehabilitation Training For The Patients With Acute Myocardial Infarction Primary After Percutaneous Coronary Intervention

Objective:Cardiovascular diseases are the leading cause for death in China or even in the whole world.Even though the emergent percutaneous coronary intervention(PCI)can save lives of patients with myocardial infarction,rehabilitation after intervention operations is complicated and takes a very long time.Through contrastive analysis on the rehabilitation effects of rehabilitation programs both in hospital and at home,this research explores a set of exercise programs that are scientific,simple,feasible and suitable for Chinese conditions,for the rehabilitation of patients with myocardial infarction after emergent intervention operations,providing bases for generalization of clinical application.Methods:Patients who met following conditions were accepted as participants of the research:through diagnosis,consistent with standards of "Guidelines for Diagnosis and Treatment of Acute Myocardial Infarction" issued in 2001 by the cardiovascular disease branch of Chinese Medical Association;subjected to emergent intervention treatment;willing to participate in the research;with the limb function and cardiac function of level 1-2,without language communication disorders and with writing abilities.Such qualified participants were classified into the control group and the observation group based on treatment conditions.Family rehabilitation was applied in the observation group while inpatient rehabilitation was applied in the control group.Individualized exercise programs were formulated based on various indexes of the exercise intensity obtained after exercise tests.The rehabilitation training period was set to be from the 7th week to the 12th week after intervention operations.Indexes of cardiac rehabilitation for both groups,such as the left ventricular ejection fraction(LVEF),the left ventricular end diastolic dimension(LVDd),6-min walking tests,electrocardiogram(ECG)and heart rate(HR),were respectively measured 2 weeks and 6 weeks after rehabilitation exercises were conducted.Measurement data were organized and analyzed through the SPSS 17 statistical software,and expressed with mean values ± standard deviations(x±s).Results:69 patients were involved in experiments of this research.After strict section according to acceptance standards,65 patients were finally selected for the analysis of this research,among which 32 patients were in the observation group(with family rehabilitation programs)and the rest 33 were in the control group(with inpatient rehabilitation programs).Both groups were comparable in terms of general conditions such as age,gender and weight.Analysis results:(1)LVEF values measured 2 weeks after exercising show that such data from the two groups have significant difference(P<0.05);LVEF values measured 6 weeks after exercising show that such data from the two groups have significant difference(P<0.05).It is indicated that the family rehabilitation is good to the left ventricular cardiac function as well as the electrocardiogram stability,leading to improvement of patients,cardiac functions as well as enhancement of patients’ living quality.(2)6-min walking tests conducted 2 weeks after exercising show that 6-min walking distances of both groups have significant difference(P<0.05);6-min walking tests conducted 6 weeks after exercising show that 6-min walking distances of both groups have significant difference(P<0.05).It is indicated that exercise abilities and endurance of patients are significantly improved after the rehabilitation exercises.(3)HR values were respectively measured 2 weeks and 6 weeks after exercising,and results show that heart rates of patients from the observation group and the control group have significant difference(P<0.05).It is indicated that the comfortable sensation of patients with simple rehabilitation exercises is improved significantly.(4)LVDd values were respectively measured 2 weeks and 6 weeks after exercising,and results show that LVDd of patients from the observation group and the control group have no significant difference(P>0.05).It is indicated that the rehabilitation of LVDd takes a long time and needs a long-term observation.(5)By comparing LVEF values and 6-min walking distances of patients from the observation group,respectively measured 2 weeks and 6 weeks after exercising,it is found that LVEF values and 6-min walking distances measured 6 weeks after exercising have significantly improved,which indicates that persistent rehabilitation exercising can significantly improve patients’ left ventricular function as well as exercise abilities;by comparing LVDd and HR values respectively measured 2 weeks and 6 weeks after exercising.statistic analysis shows no significant difference(P>0.05).which indicates that relevant cardiac function of patients can be maintained after 6 weeks of exercises with effective intensity.(6)By comparing LVEF values and 6-min walking distances of patients from the control group,respectively measured 2 weeks and 6 weeks after exercising,statistic analysis shows significant difference(P<0.05)while by comparing LVDd and HR values respectively measured 2 weeks and 6 weeks after exercising,statistic analysis shows no significant difference(P>0.05).Both the control group and the observation group have similar results.It is indicated that rehabilitation exercises can improve the coronary blood flow,accelerate the formation and opening of collateral circulation between avascular necrosis cardiac muscles and normal cardiac muscles,and enhance blood flow reserve of coronary arteries.Conclusion:Through formulating proper individualized exercise programs and by comparing rehabilitation effects of cardiac function of patients with acute myocardial infarction,after such patients are subjected to intervention operations and then treated with family rehabilitation programs and inpatient rehabilitation programs,it is believed that family rehabilitation programs are better than inpatient rehabilitation programs within the observation period,worthy of continued clinical observation and practice.
