The Treatment And Prevention Of Complications Related Gas In The Poem For Achalasia Of Cardia

Achalasia is a motility disorder of the lower esophagus,also called cardiac spasms.POEM is the abbreviation of myotomy peroral through the endoscopic resection of the esophagus(endoscopic).Japanese scholar Inoue operated in the human body successfully for the first time and alleviated the symptoms of patients with esophageal obstruction.With the development,the incidence of complications rised rapidly.Complications related gas is caused by gas through the tunnel into the esophagus wall cavity,including pneumothorax,mediastinal emphysema,pneumoperitoneum and subcutaneous emphysema.There are a lot of experts dedicated to the complications related gas,but there is no specific articles dedicated the conclusive advice about current treatment and prevention.Here is a case of POEM surgery complicated with pneumothorax,pneumoperitoneum and mediastinal emphysema.Although the surgery patients in critical condition,but after timely diagnosis and treatment,the prognosis was good.Through this case,I found that as an anesthesiologist,there is need to timely discovery,timely diagnosis and timely treatment.Therefore,to review the literature of POEM for achalasia of the cardia,I summarized the etiology,pathophysiology,diagnosis,treatment and prevention.I put forward how to prevent the occurrence of complications related gas and reduce the incidence of complications related gas.
