Objective: To provide a theoretical basis for early clinical diagnosis of rheumatoid arthritis synovial membrane lesions,the high frequency ultrasound and contrast-enhanced ultrasound(Contrast-enhanced UItrasonography,CEUS)is applied to keep continuous and dynamic observation of rheumatoid arthritis(Rheurnatoid Arthritis,RA)in the animal model of early synovial membrane and blood perfusion.Methods: The collagen-induced arthritis(CIA)model is made from chicken type II collagen and incomplete adjuvant arthritis rats.After making,the five-grade scoring system is used for the degree of joint inflammation,which calculates arthritis inflammation index.The score of the joint is equal to 3 points or more than 3 points will be regarded as the experimental object.The 70 feet of 35 rats that are observed by eyes will be recorded as successful model.The ankle synovial thickness and synovial blood supply are observed by high frequency ultrasound,the synovial thickness was recorded,and the blood supply was graded.Each foot of the 35 rats that tail vein were injected with SonoVue type projectile were observed,the hind side of synovium will be chosen as the region of interest(Region of Interest,ROI),the time intensity curve(Time Intensity,Curve,TIC)analyzed by the quantitative analysis software,the peak(Peak),local blood flow volume(Regional Blood,Volume,RBV),blood flow(Regional Blood Flow,RBF)were recorded.The parameters of the above items will be observed for 4 weeks,and then compared the results of each group,which will be taken as statistically significant difference.Result:1.Model arthritis index scoreThe average score of arthritis index on the 14 day of the model 35 rats' hind feet was 2.5,and the results were as follows: the average of 21 days was 8.8,the average of 28 days was 8.0,the average of 35 days was 7.6,the average of 35 days was 7.6;the average of 42 days was 4.6,the peak of arthritis index score was between the 21 day and the 35 day,then decreased on the 42 day.2 Synovial thicknessThe thickness of synovial was 2.8±1.2mm on the 21 day,3.9±1.4mm on the 28 day,4.8 ± 1.7mm on the 35 day,and 2.7±1.5mm on the 42 day,from primary immune gradually increased to on the 21 day,reached the peak on the 35 day,decreased on the 42 day.The synovial difference between the 28 day and the 21 day,the 35 day and the 28 day,the 42 day and the 35 day being statistically significant(p<0.05).3 Ranking system of synovial flowAfter 21 days,rank 1 of the synovial blood of ankle join was 52,rank 2 was 18,rank 3 was 0;after 28 days,the rank 1 of the synovial blood of ankle join was 12,rank 2 was 38,rank 3 was 20;after 35 days,the rank 1 of the synovial blood of ankle join was 8,rank 2 was 41,rank 3 was 21;after 42 days,the rank 1 of the synovial blood of ankle join was 36,rank 2 was 28,rank 3was 6.The synovial blood flow signal can be observed after 21 days,the synovial blood flow increased significantly on the 28 and 35 day,and decreased on the 42 day;the synovial blood supply of the 28 day increased more than that on the 21 day,which the same as that on the 35 day and the 28 day,the difference being statistically significant.(p<0.05),.The difference between the 35 day and the 28 day being no statistically significant(p>0.05).4.Comparison of post-processing parameters of contrast-enhanced ultrasoundThe contrast time intensity curve(TIC)parameters of rheumatoid arthritis rat model's 35 feet : the peak of 21 days was 26.5±3.2,The Regional Blood Volume(RBV)was 845.42±150.42,the Regional Blood Flow(RBF)was 34.62±2.61;the peak of the 28 day was 36±3.7,The Regional Blood Volume(RBV)was 947.6±172.35,the Regional Blood Flow(RBF)was 45.03±3.13;the peak of the 35 day was 43.6±3.6,The Regional Blood Volume(RBV)was 1108.16±177.53,the Regional Blood Flow(RBF)was 55.08±4.13;the peak of the 42 day was 32.5±2.9,The Regional Blood Volume(RBV)was 885.42±173.86,the Regional Blood Flow(RBF)was 38.57±3.93;the parameters gradually increased from the 21 day,reached the peak at 35 days,decreased on the 42 day.The data of the 28 day compared with the 21 day being statistically significant(p<0.05),the same as the 35 day compared with the 28 day and the 42 day compared with the 35 day.Conclusion:1.The high frequency ultrasound and contrast enhanced ultrasound in the observation of the pathological changes of synovium and synovial flow in rheumatoid arthritis rats is of high value.2.The parameters of contrast-enhanced ultrasound can be more sensitive to the changes of blood perfusion.3.This experiment proved that the collagen of rat model of rheumatoid arthritisto show the natural inflammatory pathological process of improvement after the peak without any treatment intervention,the results also provide a certain reference value for the experiments.
