A Clinical Comparative Study Of Single-Portal VATS And Multiportal VATS Thoracoscopic Lobectomy For Early-Stage Non-Small Cell Lung Cancer

Objective:Through the comparison of three different surgical methods,to investigate the clinical effect and application value of single-portal video-assisted thoracoscopic surgery lobectomy for non-small cell lung cancer.Methods:A retrospective analysis was performed on 90 patients with early stage non small cell lung cancer treated by video-assisted thoracoscopic lobectomy in the Thoracic Surgery 1,Second Affiliated Hospital of Dalian Medical University,from November 2014 to December 2016.Among them,30 cases underwent single-portal video-assisted thoracoscopic lobectomy(single-portal VATS group),30 patients underwent single utility portal video-assisted thoracoscopic lobectomy(double-portal group),and 30 cases underwent 3-portal video-assisted thoracoscopic lobectomy(3-portal VATS grop),90 cases of preoperative clinical stage were I stage.By statistical comparison of three groups of patients with operation time,intraoperative blood loss,number of lymph node dissection group,postoperative catheter time,postoperative third day pain score,postoperative complications,hospitalization days and total hospitalization expenses,self-reported surgery satisfaction.Results:there was no significant difference between the three groups in gender,age,pathological types,pathological stage and other clinical data(P>0.05).And in the number of lymph node dissection group,postoperative complications,hospitalization costs,patients satisfaction of private prosecution operation was not statistically significant.However,there was statistical significance in the operation time,intraoperative blood loss,short indwelling time,postoperative pain score third days after operation,and postoperative hospitalization time were less than those in the double-port group and the 3-portal group(P<0.05).Conclusion:single-portal VATS lobectomy for early non-small cell lung cancer than the other two kinds of minimally invasive surgery,which has more obvious trauma,less postoperative pain,quick recovery and minimal invasive,it is worthy of clinical application..Its advantages and disadvantages coexist,However,it is necessary to combine the characteristics of each patient to choose minimally invasive surgery,the use of individualized minimally invasive treatment program.
