Pneumonia is the first of the four common diseases of children all of the world,it has a high morbidity and mortality,especially infants and young children of common and frequently-occurring disease.What is more severe pneumonia is a high resource consumption and high mortality disease,Is the main cause of child mortality,Which produce to the family and society a great burden.Therefore,how to prevent,detect and treat children with severe pneumonia has become one of the urgent problems to be solved in pediatrics.Children with severe pneumonia etiology research show that the main pathogenic bacteria.Early and rational use of antibiotics can effectively reduce the mortality rate of children.Therefore,early empirical use of antibiotics throughout the course of treatment Is particularly important.In this study,from January 2015 to December 2016 Changchun City Children's Hospital hospitalized 166 cases of children with severe pneumonia were etiological,drug sensitivity and drug resistance test,and analysis of clinical data.Objective: To understand the distribution of pathogens in children with severe pneumonia and their sensitivity to common antibiotics,and to provide evidence for clinical prevention and treatment of severe pneumonia in children,improve the survival rate,improve the quality of life and reduce the mortality rate.Methods: Study subjects: 166 cases of clinical diagnosis of severe pneumonia in Department of Respiration,Changchun Children's Hospital.Study methods: to collect sputum and blood specimens;Application of bacterial culture and ELISA for pathogen detection,detecting pathogens include bacteria,viruses,mycoplasma,chlamydia and fungi,and drug sensitive test of some bacteria.Results: Among the 166 cases,there were 131 cases with pathogens positive.Pathogen infection rate was 78.92%,35 cases(21.08%)were pathogen free.Among them,58 cases(44.28%)were purely bacteria infection,42 strains were Gram-negative bacteria,mainly Klebsiella pneumoniae and Pseudomonas aeruginosa,12 strains(20.69%)and 9 strains(15.52%)respectively;16 strains of Gram-positive bacteria,mainly constructed of Staphylococcus aureus and Streptococcus pneumoniae,were 8(8.63%)and 5(5.17%)respectively.Thirty cases(22.90%)were detected to virus,mainly Respiratory syncytial virus and Adenovirus were 12 cases(40.00%)and 8 cases(26.67%)respectively.Fungus was detected in 4 cases,(3.05%),are all Candida albicans.Mycoplasma detection in 16 cases(12.21%),mixed infection was 23 cases(17.56%).There were 37 cases(22.29%)in spring,29 cases(17.49%)in summer,39 cases(23.49%)in autumn and 61 cases(36.75%)in winter.In this study 34 strains of Gram-negative bacteria and 11 strains of Gram-positive bacteria were tested with 10 and 8 kinds of antibiotics commonly used antibiotics and found that Klebsiella pneumoniae was susceptible to piperacillin / tazobactam,levofloxacin,imipenem,ciprofloxacin,minocycline.Pseudomonas aeruginosa was susceptible to gentamicin and levofloxacin;E.coli susceptibility to piperacillin / tazobactam,minocycline,sulfamethoxazole and imipenem.Acinetobacter baumannii is sensitive to minocycline;Staphylococcus aureus is highly sensitive to vancomycin;Streptococcus pneumoniae is highly sensitive to chloramphenicol,penicillin,ceftriaxone and vancomycin.There is a different degree of resistance rate for several other commonly used antibiotics.Conclusion: The incidence of severe of pneumonia is high in winter and autumn.The main pathogen of infection is bacteria.Klebsiella pneumonia is the fist,and Pseudomonas aeruginosa second.The main virus of severe pneumonia is RSV.Klebsiella pneumoniae was susceptible to piperacillin / tazobactam,levofloxacin,imipenem,ciprofloxacin,minocycline.Pseudomonas aeruginosa was susceptible to gentamicin and levofloxacin;Staphylococcus aureus is highly sensitive to vancomycin.
