Comparison Of Three Kinds Of Pressure Sore Evaluation Scale In Evaluating Pressure Sore Of Patients With Advanced Tumor

Objective: Compared with the Braden scale,the waterlow scale and the Norton scale,the effect of the pressure ulcer and the optimal threshold were determined.The risk factors of the pressure ulcer were selected for the patients with advanced tumor.And to provide theoretical basis for the development of pressure ulcer assessment scale in patients with advanced stage of tumor.methods: Selected from January 2016 to December 2016 to stay in Xinjiang,a tertiary tumor hospital tumor patients with advanced cancer as the object of study.For patients whomet the inclusion criteria during the study period,the researchers used the Braden evaluation form,the Waterlow evaluation form,and the Norton assessment table.The pressure ulcer assessment table was evaluated and investigated at 24 hours after admission,followed by daily skin observation,Until the patient has pressure sores,discharge,transfer or death.When pressure ulcers occur,record the location of the pressure sores,staging,time of occurrence.Data were analyzed using SPSS 22.0 statistical software andmedcalc15.2 statistical software.Results: 1)There were 14 cases of patients who did notmeet the criteria and repeated admission.There were 416 cases of patients whomet the inclusion criteria and 57 cases of pressure sores.The incidence of pressure ulcer was 13.70%.57 cases of pressure ulcers occurred in patients with pressure sores 57,? pressure sores 50(87.72%),? pressure sore 7(12.28%),pressure ulcers high parts of the sacrococcygeal,followed by the anterior superior iliac spine.2)Brady scale of the six dimensions and total partial pressure sore group was lower than the non-pressure sore group,the waterlow scale in age,skin type,tissue malnutrition,incontinence,athletic ability,appetite,drug therapy 7 dimensions and total partial pressure Sore group was higher than non-pressure sore group;Norton scale 5 dimensions and total partial pressure group was lower than non-pressure sore group.3)The Braden evaluation scale was used to evaluate the risk factors of the patients with advanced tumor.The results showed thatthe diagnostic value of the evaluation scale was 16 minutes,and the corresponding sensitivity,specificity and positive predictive value,Negative predictive value,respectively 91.23%?85.24%?49.50% and 98.40%.4)Using the Waterlow evaluation scale,the risk factors of the patients with advanced tumor were evaluated.The results showed that the diagnostic value of the evaluation scale was 14 points,and the optimal value of the sensitivity,specificity,positive predictive value The predicted values were94.74%?81.34%?44.60% and 99.00%respectively.5)Using the Norton evaluation scale,the risk factors of the patients with advanced tumor were evaluated.The results showed that the diagnostic value of the scale was 14 points,and the corresponding sensitivity,specificity,positive predictive value and positive predictive value were Was 84.21% ?79.94%?40.00% and 97.00%respectively.6)The area under the ROC curve of the Braden scale,the waterlow scale,and the Norton scale were 0.847?0.904?0.845,respectively.Youden index was 0.768?0.761?0.642.Conclusion: 1)Patients with advanced cancer is a high risk of hospital-induced pressure sores,medical staff to timely assessment of the patient's skin condition,and to the appropriate preventivemeasures to the pressure parts of the decompression,to prevent and reduce the occurrence of pressure sores.2)The cognitive ability,humidity,activity,mobility,nutritional intake,frictional force and shear force of the Braden scale were related to the occurrence of pressure sore,and the lower the scores were Low risk of pressure ulcers;the age,skin type,tissuemalnutrition,incontinence,athletic ability,appetite,and drug therapy in the waterlow scale,the seven dimensions and the total score of Waterlow were related to the occurrence of pressure ulcer The higher the score,the higher the total score,the higher the risk of pressure sore;Norton scale in the general state,the state ofmind,activity,walking,incontinence 5dimensions and Norton score and pressure ulcer occurred,the dimension score The lower the total score,the higher the risk of pressure ulcers.When using the pressure ulcer risk assessment tool,should be accurate to determine,accurate assessment.3)By comparing the relevant evaluation index,the Braden scale,the waterlow scale and the Norton scale were used to evaluate the risk factors of pressure ulcer in patients with advanced stage.The sensitivity and specificity of the three evaluation scales were In contrast,the sensitivity,specificity,the Youden index and the area under the ROC curve were higher and the application effect was better when the diagnostic value of the waterlow scale was14 points.
