Objective: To investigate the best way to totally resect the intra-and extra-spinal tumors and maintain the spinal stability through instrumentation.Methods:Cinical date of 12 patients with intra-and extra-spinal tumors resected by microsurgery combined with spine instrumentation from November 2014 to November 2015.Spinal instrumentation via back approach should be used to protect the stability of the spinal.Spinal instrumentation via posterior middline approach was performed for fusion of lesion segments after tumor resection in 10 cases.Results: Tumors were totally removed in 9 cases and subtotally removed in 2 cases.Pathological report: schwannoma 8 cases,neurofibroma 1 case,Malignant mesothelioma 1 case,Angiosarcoma 1 case.Malignant peripheral nerve sheath meningioma 1 case.The Neurological were improved in 11 cases 15 days after surgery.Unchanged in 1 case.No worsen cases.The patients were followed-up for 18 to 30 months with an average of 25 months.No tumor relapse and spinal malformation were found.Conclusion: Most cases of dumbbell intra-and extra-spinal tumors can be treated with one-stage microsurgery.and use spine instrumentation to maintain the spine stability.
