Objective To evaluate the diagnostic performance of 3D ASL MR perfusion imaging technology in preoperative diagnosis classificationand postoperative brain tumorsincreased reactivityand relapse.Methods We examined images form 53 patients with brain tumors and 19 patients with suspected recurrence of gliomas after operation who underwent ASL and CE MR imaging. Selected 19 cases of cerebral gliomas after operation were confirmed by the secondary surgical pathology or follow-up confirmed as glioma recurrence or gliosis. We then analyzed imaging findings of brain tumors,recurrent glioma and gliosis. The maximal cerebral blood flow of solid regions of tumor and the cerebral blood flow of contralateral hemisphere were measured on maps. T test statistical analysis was carried out on the CBF and rCBF.These parameters of both groups were compared and ROC curves were drawn to determine the diagnostic thresholds. The tumor areas were measured by the two methods respectively. T test statistical analysis was performed to analyze the relationship between the measurements of tumor areas.Results 3D ASL perfusion imaging rCBF quantitative parameters were statistically significant between the low grade gliomas with high grade gliomas(P < 0.01 ) and meningiomas(P < 0.01 ) and cerebral metastatic tumors(P<0.01) , but revealed no statistically significantly difference between grade gliomas and meningiomas and cerebral metastatic tumors . In ASL perfusion image, part of the high grade glioma showed obviously high perfusion,low grade glioma showed low blood perfusion. Buthigh signal intensity on ASL is sometimes seen outside the CE area. The differences of low grade gliomas areas acquired form ASL-CBF and the routine enhanced MR image were significance(P < 0.01 ) .The differences of high grade gliomas and meningiomas and cerebral metastatic tumors areas acquired form ASL-CBF and the routine enhanced MR image were significance(P>0.05). The rCBF values was significantly higher than that of radiation-induced brain injury group , and have significant statistical difference between the two.Conclusion 3D ASL can quantitatively react tumor microcirculation perfusion with conventional MRI sequences in combination, can be used as an important supplement for brain tumor diagnosis and after surgery grade gliomas have important reference value.
