Comparisons Of GnRH Antagonist Versus GnRH Agonist Protocol For IVF/ICSI-ETt In Adwanced Age Women With Normal Ovarian Reserve

Objective: To analyze the clinical efficacy of GnRH agonist protocol and GnRH antagonist protocol in advanced age(age?35y),normal ovarian reserve,infertile women with different age.Methods: From December 2014 to March 2016,retrospective analysis of advanced age infertile wome with a total of 212 cycles were included using the IVF/ICSI-ET treatment in the Center of Guangxi Reproductive Medicine,the First Affiliated Hospital of Guangxi Medical University,According to the different ovarian Hyperstimulation protocol,two groups were divided(group GnRH-a and group Gn RH-ant).According to the age,three subgroup analysis were conducted,(35y?age<38y,38y?age<41y,age?41y;group:GnRH-a1,GnRH-a2,GnRH-a3;GnRH-ant1,GnRH-ant2,GnRH-ant3)to analyze the clinical efficacy for Gn RH agonist protocol and Gn RH antagonist protocol in advanced age patietns.Results:For the group Gn RH-a and Gn RH-ant,Significant statistical differences were found between the oocytes and enbryos,high quality embryos(P < 0.05),No statistical differences were found for the implantation rate clinical pregnancy rate(P>0.05).GnRH-a1 recieve more oocytes and embryos,higher implantation rate and clinical pregnancy rate than GnRH-a3(P <0.05).For the subgroup GnRH-a1,GnRH-a2,GnRH-a3andGnRH-ant1,GnRH-ant2,GnRH-ant3.Significant statistical differences were found between the duration and dosage of gonadotrop in(Gn)Administration(P<0.05).No statistical differences were found for the implantation rate clinical pregnancy rate(P>0.05).For the subgroup GnRH-a1 and GnRH-ant1,Significant statistical differences were detected between oocytes,embryos,high quality embryos(P<0.05).For the subgroup GnRH-a2 and Gn RH-ant2,Significant statistical differences were detected between oocytes,embryos(P < 0.05).No statistical differences were detected between the high quality embryos(P>0.05).For the subgroup Gn RH-a3 and GnRH-ant3,There weren't statistical differences between oocytes,embryos,high quality embryos(P>0.05).Conclution:1.For patients old than 35 years with normal ovarian reserve,both the two protocol have oocytes and embryos decreasing,more dosage of gonadotropin(Gn)Administration using,implantation rate and Clinical pregnancy rate decreasing with age.2.The two protocol have the same clinical resuilts in patients older than 35 years,but GnRH antagonist protocol has the benefit on decreasing the dosage of gonadotropin(Gn)Administration,Shorten Treatment cycle,injection times,Improving the acceptances of patients.
