Objective: The detection of CD137(4-1BB),CD137L(4-1BBL)expression in pancreatic cancer group and adjacent normal pancreatic tissues,and the expression of CD137 L in pathological stage and prognosis of pancreatic cancer and the level of CD137 and CD137 L,explore the impact of immune microenvironment of pancreatic cancer.Methods: Wangnan Medical College affiliated yijishan Hospital Department of general surgery after surgery by standard pathology diagnosed with pancreatic cancer in 42 cases of paraffin embedded specimens collected,and collected the corresponding adjacent normal pancreatic tissue specimens were collected from May 2012 to October 2015 were time.The paraffin specimens in Wangnan Medical College yijishan hospital by immunohistochemistry(EnVision two steps)to detect the expression of pancreatic carcinoma and adjacent normal pancreatic tissue in CD137 and CD137 L,to analyze the clinical and pathological characteristic(tumor size,location,there is no neurovascular invasion,no lymph node metastasis,differentiation and clinical stage of the relationship).Collect the clinical data of 42 patients with pancreatic cancer(gender,age,tumor system diseases have no history,no radiotherapy and chemotherapy,etc.)and visit(outpatient and telephone)of the 42 patients with survival time(survival time of patients after standard: from the first day until the date or last visit to the world the date of October 2016),retrospectively analyzed the relationship between the expression of CD137 L and CD137 in patients with pancreatic cancer clinical pathological parameters and prognosis.Results: CD137 was not expressed in pancreatic cancer tissues(0/42),and was not expressed in normal pancreatic tissues(0/42),but was partially expressed in vascular endothelial cells(11/42)in pancreatic cancer tissues.CD137 L in pancreatic cancer was mainly expressed in cell membrane and cytoplasm,no expression in adjacent normal pancreatic tissues,42 cases of pancreatic cancer CD137 L positive expression in 26 cases,with a significant difference(P<0.05).42 cases of pancreatic cancer were divided according to gender,age,tumor size,clinical stage,degree of differentiation,neural invasion,vascular invasion,tumor location,lymph node metastasis group(I+II),results showed that the positive expression of pancreatic cancer was higher than that of pancreatic cancer(III+IV)(P= 0.003),high / positive expression in differentiation pancreatic cancer was higher than that of low/low differentiated pancreatic carcinoma(P=0.015),and with other clinicopathological parameters(gender,age,tumor location,tumor size,lymph node metastasis,no nerve vascular invasion)no significant correlation(P>0.05).The median survival time was 13 months and 42 pancreatic cancer patients,the median survival time was 18 months in CD137 L positive expression and negative expression of CD137 L had a median survival of 6 months,the survival time compared with Log-Rank method,the positive expression of CD137 L had a better prognosis(P=0.021).Cox regression multivariate analysis showed that clinical stage(P=0.002),vascular invasion(P=0.003)were independent prognostic factors affecting the survival of pancreatic cancer patients,CD137 L,age,gender,tumor size,growth and differentiation,no independent prognostic factors for survival of neural invasion and lymph node metastasis were not surgery.Patients with pancreatic cancer(P>0.05).Conclusion: CD137 was not expressed in pancreatic cancer tissues,and was not expressed in normal pancreatic tissues.The expression of CD137 L in pancreatic cancer was higher than that in adjacent normal pancreatic tissue.The expression level of CD137 L was correlated with the clinical stage.The expression of CD137 L was higher than that of advanced pancreatic carcinoma.The expression level of CD137 L was positively correlated with the prognosis of pancreatic cancer,and it has important value in evaluating the prognosis of pancreatic cancer.
