Primary cutaneous apocrine carcinoma is a rare clinical skin adnexal tumor,which often performances as a single or multiple painless nodules or plaquesthe in armpits,anus and genitals where large sweat gland is abundant.It is also visible in external auditory canal,eyelid,mammary gland where deformation of large sweat gland is distributed.Its etiology and pathogenesis is unclear.Its pathologic examination and the clinical manifestations are lack of specificity,and it is easily misdiagnosed as benign lesions of the skin.There are no evidence-based medicine has definite curative effect of the treatment,thus,the clinical treatment of apocrine carcinoma has not reached a consensus.Frequently,local expanded resection acts as the preferred alternative.Apocrine carcinoma has the traits which is easily local recurrence and/or lymph node metastasis.In order to delay the progress of tumor and improve the patient's quality of life,radiotherapy and systemic chemotherapy also be used in these patients.Because of its low incidence and less clinical datas,the diagnosis and treatment still exist certain differences.We present a 62-year-old male patient with with apocrine carcinoma in the right side of the perineum,which had been present for more than 6 months.Biopsy pathology examination found squamous epithelial malignant tumor,paget's disease to row.Pathological diagnosis is low differentiated adenocarcinoma and lymph node metastasis,immunohistochemical markers revealed large sweat gland differentiation.Recurrence and metastasis has been identified after one months later.The patient has received chemotherapy which is combination of cyclophosphamide and pharmorubicin and capecitabine.After two cycles treatment,efficacy evaluation is stable disease(small).However,cardiac ejection fraction decreasing has been observed after three cycles treatment.For security reasons,we adjusted paclitaxel plus cisplatin regime as a adjuvant therapy.In this paper,we reporte the case and combining with literature review is as follows.
