Background:Adverse transfusion reactions are the most common adverse events in the transfusion process.Although the majority of blood transfusion adverse reactions were mild,but may still cause medical cost in patients with increased or treatment time prolonged,the burden of patients and their families and society;and in a few cases,adverse reactions of transfusion patients will lead to serious complications and even death.Therefore,the monitoring of adverse reactions of blood transfusion is highly valued.However,due to the large hospital transfusion case,if in each case of blood transfusion adverse events are identified for any large general hospitals are not operational,because it requires additional more professional personnel.At present,the identification of adverse transfusion reaction is reported in the physician in charge of the case of blood transfusion,blood transfusion department due to the physician in charge of professional knowledge or limited because of the busy business and the inevitable omissions,false positives and false negatives,which leads to the distortion of the data of adverse transfusion reaction.In view of this,this study will use the electronic medical record system of the hospital,through the analysis of the history of blood transfusion cases,trying to strip out characteristic variables can reflect the adverse reactions of blood transfusion,and as a screening key word,by logical judgement,screened potential adverse reactions to blood transfusion,blood transfusion adverse reactions confirmed by the expert committee.This can greatly reduce the blood transfusion adverse reaction of the Committee of experts identified the workload,and can greatly reduce the false negative rate possible,so that the monitoring of adverse reactions of blood transfusion can significantly improve the accuracy and maneuverability.Objectives:This paper presents a set of screening strategies based on hospital electronic medical records system to screen for adverse events,so as to improve the accuracy and feasibility of the monitoring of adverse transfusion reactions.Object of study:A total of 31 hospitalized patients with blood transfusion in a southern hospital from January 1,2014 to 2016 at the age of 05 days were selected as subjects.Methods:According to the patient,and blood transfusion to determine the injection event information,using PowerBuilder9.0 development tools,through structured query language SQL,with the condition of blood transfusion adverse reactions in the Orcal database query keywords,nursing records and electronic medical records system and three token record,screening records of transfusion related data,such as the results of screening is not empty the preliminary judgment of potential adverse events,blood transfusion.The corresponding description of the monitoring period before and after the blood transfusion was combined with the results of three time screening,and the potential adverse transfusion adverse events were reduced according to the principle of potential blood transfusion adverse events.On the basis of the above,we should search for the doctors' intervention measures,and give the screening potential adverse events according to the search results.Adverse reactions Expert Committee for the potential adverse transfusion events,combined with the patient's course of disease records,medical orders and related inspection information to confirm the adverse reactions of transfusion.The rate of blood transfusion was calculated according to the four units of blood transfusion,blood transfusion,blood transfusion and blood transfusion.Results:There was a significant difference in the rate of adverse transfusion reactions and the rate of adverse reactions between the two groups(P<0.001).Conclusion:A new method of screening for adverse transfusion reaction was proposed.
