Clinical Analysis Of High Intensity Focused Ultrasound In The Treatment Of Adenomyoma

BackgroundAdenomyoma is common benign tumors of the female reproductive system and are tumors that seriously affect women's health.Occur in the 30-50 years of age by maternal,adenomyoma's clinical symptoms are far more seriously than the uterine fibroids.The main clinical symptoms are progressive increase in dysmenorrhea,menstrual abnormalities,prolonged menstrual period and so on.The increase in menstrual flow will lead to anemia,malaise,infertility,systemic diseases of the system.The pathological basis of adenomyoma is that the endometrial which has function invaded into the myometrium,with the menstrual period bleeding that the myometrium thickening and forming focal lesions,the gradual development of intramural myoma with the serious symptoms such as dysmenorrhea or others.The incidence of adenomyoma was 8.8%-31.0 % [1],and the incidence rate gradually increased.In the past,the treatment of adenomyoma is more invasive,and now patients are more and more demanding for a treatment which is conservative to retain the uterus.High Intensity Focused Ultrasound(HIFU)is a new noninvasive method for the treatment of solid tumors in recent years.It is now widely regarded as a conservative treatment for the treatment of adenomyoma.This study was to evaluate the efficacy of HIFU in the treatment of adenomyoma by compared with the volume of adenomyoma treated with HIFU before treatment,changes in dysmenorrhea,changes in menstrual amount and changes in CA125.ObjectiveTo study the clinical effect of High Intensity Focused Ultrasound in the treatment of adenomyoma.MethodsSelection of the object for the September 2014-September 2016 in Xinxiang Medical College First Affiliated Hospital of patients treated,which the diagnosis of adenomyoma is clear.All patients had the symptom that menstruation increased and dysmenorrhea symptoms,and have the desire to retain the uterus,willing to accept High Intensity Focused Ultrasound to treat the adenomyoma.Treatment: The equipment used is Chongqing Haifu(HIFU)Technology Co.,Ltd.that developed focus ultrasound tumor treatment system,model is JC200,In the operation process,60 cases(a total of 64 adenomyoma)patients under sedation analgesia treatment.In the use of real-time ultrasound sonography guide.3 and 6 months after treatment compared with before treatment by asking patients to change the clinical symptoms and adenomyoma in the image of the changes under the test to evaluate the efficacy of HIFU to treat the adenomyoma.ResultsClinical data of patients before treatment with HIFU: In this study,the size of adenomyoma before treatment was 34.85 ± 17.89 cm3;Dysmenorrhea score of 4.47 ± 2.04 points;Patients before treatment of menstrual amount based on 100%,menstrual amount expressed as 100 ± 0 %;The value of CA125 was 129.50 ± 142.03 U / ml.Clinical data at 3 months after treatment: the size of adenomyoma were 29.27 ± 14.29 cm3;Dysmenorrhea score was 2.98 ± 1.48;Menstrual flow was 68 ± 12%;The value of CA125 was 111.63 ± 111.01 U / ml.Clinical data at 6 months after treatment: the size of adenomyoma were 28.88 ± 11.65 cm 3;Dysmenorrhea score was 1.98 ± 1.03;Menstrual amount was 52 ± 14%;The value of CA125 was 100.72 ± 103.32 U / ml.Before treatment,respectively,compared with 3 and 6 month after treatment by using q test.Can be obtained that the adenomyoma less than the volume before treatment,dysmenorrhea score lower than before and the symptoms reductive than before,menstrual flow is also significantly less,CA125 reduction is less than before after 3 and 6 months treatment.All the differences were statistically significant.In the selected 60 patients,fewer complications occurred.Only one patient with symptoms of right lower limb numbness;After 3 months of using of nutrition drugs(VitB1,VitB12)the symptoms disappeared.Only one case of skin burning,half months after the symptoms disappeared.12 patients after accepting the treatment of HIFU had the emergence of lower abdominal pain symptoms,and then the symptom relief after using the drug of analgesic,The pain symptoms completely disappeared after a week.ConclusionThe treatment of High Intensity Focused Ultrasound to adenomyoma is safe and effective,can effectively reduce the size of adenomyoma,improve dysmenorrhea symptoms,reduce menstrual amount,but also to a certain extent,reduce the laboratory test indicators.It can be a viable choice for patients who wants conservative treatment because of the fewer complications.
