Objective:To explore the expressions of carbonic anhydrase ?(CA?)and its mRNA contents in the quadriceps femoris muscles and intercostal muscles of the control subjects and chronic obstructive pulmonary disease(COPD)patients with different severity classification.Methods:The quadriceps femoris muscle tissues of 75 patients who received operations in the Orthopedics department of the first affliated hospital of Kunming Medical University were collected,including 25 normal controls,24 mild COPD patients and 26 moderate COPD patients.The intercostal muscle tissues of 72 patients who received operations in the department of thoracic surgery of the first affliated hospital of Kunming Medical University were collected,including 23 normal controls,24 mild COPD patients and 25 moderate COPD patients.A total of 147 samples were divided into 2 groups:quadriceps femoris muscle group(Q group)and intercostal muscle group(C group).Each group was then divided into 3 subgroups based on the different severity classification of COPD:control group(FEV1/FVC>70%,FEV1%(FEV1 predicted)?80%),I group(FEV1/FVC<70%,FEV1%(FEV1 predicted)?80%)and ? group(FEV1/FVC<70%,30%?FEV1%(FEV1 predicted)<80%).The CA? and CA? mRNA were measured using ELISA and RT-PCR and compared between different groups.Results:There is no significant difference between the Q control group and the C control group in the concentration of both CA? and CA? mRNA.Statistical differences were found when the comparisons of the concentration of CA? carried out between the Q I and C I groups(12.10±1.10,11.03±0.90;t =3.69,P<0.001)and the Q?I and C? groups(15.40±1.05,13.52±0.91;t=6.85,P<0.001).For the concentration of CA? mRNA(2-??Ct),no significant difference was found between Q I and C I groups(2.18±0.95,2.10±0.84;t =0.305,P =0.76),but there was difference between the Q?I and C? groups(3.79±1.03,3.01±0.82;t =2.96,P=0.005).For subgroup analysis within the Q group,the CA?(ng/ml)and CA?mRNA(2-??Ct)were significant different among the Q control,Q I and Q? group(CA?(ng/ml):10.13±1.22,12.10±1.10,15.40±1.05;F=143.85,P<0.001);CA? mRNA(2-??Ct):1.00±0.44.2.18±0.95.3.79± 1.03;F=69.88,P<0.001).The subgroup analysis within the C group showed that the CA? and CA? mRNA were also significant different among the C control,C I and C? group(CA?(ng/ml):10.09±0.88.11.03±0.90.13.52±0.91;F=94.58,P<0.001);CA? mRNA(2-??C):1.00±0.43.2.10±0.84.3.01±0.82;F=45.89,P<0.001).Conclusion:CA? and CA? mRNA concentration in the skeletal muscles of COPD patients are positively related and COPD patients got a higher CA? and CA?mRNA concentration in some of the skeletal muscles,especially skeletal muscles of the lower limbs.For COPD patients at different stages,the CA? and CA?I mRNA concentration differed with the severity classification.The expression of CAIII and the transcription of CA? mRNA are influenced not only by the systemic factors,but also by local factors.
