| Nuchal cord is a common obstetric complication which incidence is high.It can also cause fetal intrauterine ischemia hypoxia that affect fetal growth.We use the Tei index and DV index to evaluate the right ventricular function of the fetuses with nuchal cord.Thus avoid the occurrence of adverse pregnancy.77 fetuses with nuchal cord(nuchal cord group)and 112 fetuses without nuchal cord(normal group)were selected from 37 th to 41 th week gestation.And the nuchal cord group was divided into S/D≥3 group and S/D<3 group,depending on the S/D ratio of umbilical artery(UA).Firstly,using TDI to measure the isovolumic contraction time(ICT),isovolumic relaxation time(IRT)and ejection time(ET)of right ventricle in each fetus,and calculate the Tei index.Then get the clear DV image through fetal abdominal transverse section or oblique section.Then,the resistance index(DVRI),pulsation index(DVPI)and S/A index of DV were obtained.Finally,the fetal right ventricle function was evaluated by the above parameters.SPSS17.0 statistical software was used.The data was expressed as mean±SD.Using analysis of variance is compared between each variable.P-value less than 0.05 is regarded statistically significant.Results showed as follows:1 There was no correlation between right ventricular Tei index and fetal heart rate(P>0.05);2 Among the age of pregnant women,fetal gestational age and fetal heart rate,there were no statistically significant difference between each group.3 Among 122 cases of normal group during fetal inspection process,8 cases of fetal because of the mother’s belly is too thick,gestational age and pillow after vertebral body larger block cardiac effects,such as haven’t obtained relatively satisfied tissue Doppler spectrum.Other 114 cases of fetal obtained clear spectrum,the success rate is about 93%.Measurement in the process of DV index,Among 122 cases of normal group,the fetuses have 7 cases due to the pillow after or obese pregnant women,can’t get a clear DV spectrum and force measurement data.Other 115 cases of DV spectrum is clear,the success rate is about 94%;4 Compared to another two groups,the fetuses of S/D≥3 group’s right ventricular Tei index and DV index are significantly higher(P<0.05);5 The fetuses of S/D<3 group’s right ventricular Tei index and DV index have no significant change with control group(P>0.05);6 The DV index of the fetuses with nuchal cord was positively correlation with right ventricular Tei index(P<0.05).In summary,the normal fetal right ventricular Tei index measurement is concise,high success rate,and not affected by the fetal heart rate,and with good stability.Joint detection of DV index,it can be more comprehensive assessment of fetal cardiac function.Compared to the group with nuchal cord,the right ventricular Tei index and DV index were lower than in normal group,the joint application of both has good sensitivity.So,the Tei index and DV index of fetuses with nuchal cord can be taken for clinical evaluation about the fetal ventricular function. |
