Effects Of Stellate Ganglion Block On Acute Lung Injury In Rats With Traumatic Hemorrhagic Shock

Abstract/Summary:
Acute lung injury(ALI)is one of the earliest organ damage after traumatic hemorrhagic shock,which plays an important role in the pathogenesis of multiple organ dysfunction and sepsis.The post-hemorrhagic shock mesenteric lymph(PHSML)is one of the important mechanisms and key links in the occurrence of ALI.It is one of great practical significance to find the treatment measures for PHSML to the prevention and treatment of ALI after hemorrhagic shock.Stellate ganglion block(SGB)has been widely used in the treatment of patients with clinical pain,and has been expanding to other disease.At present,it is not clear that SGB on severe hemorrhagic shock-induced organ damage.Cell autophagy is involved in the occurrence and development of various diseases.The role of autophagy in the pathogenesis of organ injury after hemorrhagic shock remains to be elucidated,whether cell autophagy is associated with the mechanism by which PHSML mediating ALI,it is unclear.Therefore,based on the establishment of SGB technique in rats,we observed the effect of SGB on the survival time of awake rats with traumatic hemorrhagic shock,furthermore,investigated the effect of SGB and drugs of autophagy on hemorrhagic shock-induced ALI,and revealed the effect of intravenous infusion of PHSML on SGB.As a result,the current study clear the role of autophagy in SGB attenuating hemorrhagic shock-induced ALI,in order to provide experimental data for the prevention and treatment of ALI induced by hemorrhagic shock.First of all,the establishment of SGB through stellate ganglion(SG)received injection of ropivacaine for long-acting SGB reversible simulation method;the model of traumatic hemorrhagic shock in rats was established by using the method of femoral vein catheterization.A total of 12 healthy male rats were randomly divided into two groups: shock group and shock+SGB group,the effect of SGB on the survival time and survival rate of hemorrhagic shock rats was observed.The study found that 6 rats were died within 24 h in the shock group;the survival time of the shock+SGB group was significantly longer than that of the Shock group.The results suggest that SGB has a good effect on hemorrhagic shock.Then,12 healthy male rats were used to establish the hemorrhagic shock model.After the fluid resuscitation,the PHSML was drained for the next experiment of intravenous infusion of PHSML.The 48 healthy adult male rats were randomly divided into 8 groups(n=6): Sham,Sham+SGB,Shock,Shock+SGB,Sham+SGB+PHSML,Shock+3-MA,Shock+SGB+ RAPA and Shock+SGB+PHSML+3-MA groups.According to the experimental design,the different intervention factors were performed at the moment of fluid resuscitation after hypotension for 60 minutes.At 3 h of fluid resuscitation or corresponding time-point,blood samples were collected from animal,lung tissue,preparation of bronchoalveolar lavage fluid and lung tissue biopsy,observe the morphological changes in lung tissues,lung permeability index,lung wet / dry the ratio of index,to observe the effects of satellite ganglion block on lung injury in rats with hemorrhagic shock.The study found that hemorrhagic shock caused by the structural damage of rat lung tissue in histology score,wet / dry weight ratio,lung permeability index increased significantly;SGB decreased the histological score,wet dry weight ratio and permeability index in rats with hemorrhagic shock;Which input PHSML abolished the SGB to reduce the effect of hemorrhagic shock in rat lung tissue histology score,wet / dry weight ratio,permeability index;the 3-methyladenine treatment decreased the hemorrhagic shock rats lung tissue histology score,wet / dry weight ratio,permeability index;the 3-methyladenine treatment to further suppress the negative effect of intravenous infusion of PHSML of rat lung tissue histology score,wet / dry weight ratio,permeability index;play a weak role in rapamycin SGB.Immunohistochemical staining was used to detect the expression of microtubule associated protein 1light chain 3(LC3-II)in lung tissue.The results showed that the positive expression of LC3-II in cytoplasm.There was a little positive expression of LC3-II in lung tissue of Sham group and Sham+SGB group,the positive expression of LC3-II was significantly increased in Shock group,the positive expression of LC3-II in shock+SGB group was weaker than that in Shock group,SGB and 3-methyladenine treatment decreased the expression of LC3-II in lung tissue of Shock group,the rats intravenous administration of PHSML and rapamycin abolished the effect of SGB on the expression of LC3-II,the 3-methyladenine treatment reduced the high expression of LC3-II induced by intravenous infusion of PHSML.The results showed that satellite ganglion block significantly increased in rats with hemorrhagic shock survival rate and prolong the survival time,further reduce hemorrhagic shock after ALI.The mechanism may be related to the inhibition of autophagy induced by hemorrhagic shock.
