Research Of Doctor-Patient Trust Based On Four-Quadrant Model

Since the early 80s of the last century, Chinese medical and health system have been reformed for three times. Many cross-cutting progresses have been made and people’s health has been improved remarkably. Meanwhile, many problems were accompanied with the progresses, among which the most representative are reduced doctor-patient trust, intensified doctor-patient conflict and increased medical disputes. Under the situation of medical reform,how to reconstruct doctor-patient trust and improve doctor-patient relationship are the important topics for discussion.This study applied four-quadrant model proposed by foreign researchers as theoretical framework to form a doctor-patient trust measurement scale via literature reviewing,interviewing and Delphi expert consultation method. This scale included 8 latitudes and 32 measurement items. The reliability and validity of four-quadrant doctor-patient trust measurement scale were tested by the case study of the second hospital of Tangshan. The total Cronbach’s a reliability coefficient was 0.912; Split-half reliability R was 0.863 and 0.932; content validity Pearson correlation coefficient was within 0.38 to 0.81, which were all statistically significant; construct validity RMR was 0.065, RMSEA was 0.073, χ2/df was 2.236,GFI was 0.791, which were all within an acceptable range. Finally, by combining test results and current situation, the reason of doctor-patient trust lost was analyzed and discussed. Based on that information, countermeasures for doctor-patient trust reconstruction were proposed.The established four-quadrant model doctor-patient trust measurement scale was believed to have good credibility and validity by the practical verification of the second hospital of Tangshan, which can be served as a scientific, reasonable and effective scale table for the government, health administration and medical institutes. It’s believed to have wide range of potential application in doctor-patient trust reconstruction and doctor-patient relationship improvement.
